Physicians Choice Home Health is a worker-owned Home Health Agency serving the South Bay and Southeast LA. We're growing, and we're looking for an experienced clinical leader to make sure every patient we serve gets safe, high-quality, compassionate care. You'll also have a real voice in how we deliver it, and a real stake in the outcome.

This is our agency's top clinical leadership role. As our Director of Patient Care Services (DPCS), you'll be the clinical executive designated under California Title 22 and the CMS Conditions of Participation. You'll oversee all nursing, rehabilitation therapy, and medical social work services, and manage and lead the clinicians who provide them. Business operations and growth are led by our Administrator, so you can focus fully on clinical quality, your team, and our patients.

You'll report to the CEO and lead the agency alongside our Administrator as partners, backing each other up so neither of you carries the load alone. You'll also work closely with our HR and operations teams to keep care consistent, compliant, and sustainable as we grow within our worker-owned cooperative structure.

What You Will Do

  • Clinical Leadership & Governance: Direct and evaluate all nursing, therapy, and medical social work services. Review and authorize admissions for clinical appropriateness, and write and update clinical policies as state and federal requirements change.
  • Team Supervision & Development: Supervise clinical coordinators and field clinicians directly. Conduct performance evaluations and field competency assessments, coach for growth, and lead final-round clinical interviews. Partner with our HR Lead to handle performance concerns fairly, consistently, and in line with California labor law.
  • Shared Decision-Making: Bring frontline clinicians into decisions about how we deliver care, in keeping with our cooperative values.
  • Quality & Performance Improvement (QAPI): Design and run our QAPI program, present quality data, and protect our CMS Star Ratings. Lead chart audits so that medical necessity is clearly documented.
  • Escalations & Advocacy: Be the final clinical decision-maker for complex physician concerns, clinical crises, and patient and family grievances, resolving them with sound judgment and care.
  • Regulatory Readiness: Keep the agency survey-ready and serve as the primary clinical contact for CDPH and other regulatory inquiries.

Day-to-Day
This position is based in our Torrance, CA office, Monday to Friday, 9am to 5pm, with periodic visits to patient homes in our service area for field audits. The role includes on-call availability outside business hours for clinical emergencies and regulatory inquiries. On-call time is included in the salary.

What You Bring
Because the DPCS is a state-designated role, some of these qualifications are set by California Title 22 regulations:

  • A current, active California Registered Nurse (RN) license with no board actions or restrictions
  • A Bachelor of Science in Nursing (BSN) or higher from an accredited program
  • 5+ years of RN experience, including 3 years within the past 5 years in home health, primary care, or acute care
  • At least 2 years of clinical leadership in a Home Health Agency
  • Working knowledge of California Title 22, CMS Conditions of Participation, and the California Nursing Practice Act
  • Experience supervising and developing clinical staff
  • Strong clinical judgment and calm, decisive leadership in a crisis
  • Clear, collaborative communication with clinicians, physicians, patients, families, and regulators
  • EMR proficiency (WellSky/Kinnser preferred), or the ability to get up to speed within 30 days of hire
  • Active BLS/CPR certification
  • A valid California driver's license and auto insurance for field audits
  • Clearance of a California DOJ/FBI Live Scan background check and a complete 10-year employment history, both required by the California Department of Public Health (CDPH) for this role

Nice to Haves: An MSN or a master's in nursing administration or a related field, CNML or CENP certification, 5+ years of home health clinical leadership, hands-on experience with CDPH surveys or designing a QAPI program, or experience in a worker-owned or cooperative organization.

Physical Requirements
This role combines computer-based work with time in the field. Essential functions include working at a computer for extended periods to review charts, audit EMR data, and write policy, and communicating effectively in person, by phone, and by video with staff, patients, families, and state investigators. You'll also move around the office to support staff, travel to patient homes for field audits, occasionally lift up to 10 pounds, and stay focused and steady during clinical crises and shifting priorities.
