A Day in the Life of a Heart Failure Program Director


The Heart Failure Program Director provides clinical and operational oversight for the heart failure program across inpatient and outpatient settings. This role combines advanced clinical expertise in heart failure management with strategic program development, quality improvement, and interdisciplinary leadership to optimize patient outcomes, reduce readmissions, and ensure regulatory and accreditation compliance.

Here is What You Will Be Doing:

  • Provides expert-level evaluation, diagnosis, and management of patients with acute and chronic heart failure (HFrEF, HFpEF, advanced HF)
  • Leads evidence-based care using current American College of Cardiology(ACC)/American Heart Association(AHA)/Heart Failure Society of America(HFSA) guidelines
  • Manages complex therapies including Guideline-Directed Medical Therapy (GDMT) and diuretic protocols
  • Directs design, implementation, and optimization of the heart failure program
  • Leads initiatives to reduce 30-day readmissions and improve quality metrics
  • Monitors performance indicators including length of stay, readmissions, mortality, and patient satisfaction
  • Ensures program compliance with institutional and national regulations
  • Participates in cardiovascular service line initiatives
  • Represents the heart failure program in leadership committees
  • Collaborates with cardiology, hospital medicine, pharmacy, nursing, case management, and social work
  • Serves as a liaison between inpatient, outpatient, and community partners
  • Participates in strategic planning, budgeting, and staffing decisions
  • Supports research and clinical trials when applicable

  • Ability to establish/maintain effective working relationships with patients, clinical staff, and the public
  • Excellent verbal, written and interpersonal communication skills

Here is What You Need:

  • Licensure (must have one of the following):
  • Current Advanced Practice Nurse-Certified Nurse Practitioner (APRN-CNP) from the Wyoming State Board of Nursing
  • Current Physician's Assistant (PA) License from the Wyoming State Board of Medicine
  • Current Federal Drug Enforcement Administration (DEA) licensure
  • Current Controlled Substance Registration (CSR) form the Wyoming State Board of Pharmacy
  • Five (5) or more years of cardiology or heart failure clinical experience
  • Fourteen (14) Days: Cheyenne Regional AHA RQI within fourteen (14) calendar days of start date
