About the Role:

The HCBS Program Coordinator plays a pivotal role in managing and overseeing Home and Community-Based Services (HCBS) programs to ensure that individuals receive high-quality, person-centered care in their homes and communities. This position involves coordinating with multiple stakeholders including service providers, clients, and regulatory agencies to facilitate seamless service delivery and compliance with federal and state guidelines. The coordinator will develop, implement, and monitor program policies and procedures to enhance service effectiveness and client satisfaction. Additionally, the role requires continuous evaluation of program outcomes and the identification of opportunities for improvement to meet evolving community needs. Ultimately, the HCBS Program Coordinator ensures that vulnerable populations have access to supportive services that promote independence and improve quality of life.

Minimum Qualifications:

  • Bachelor’s degree in social work, public health, healthcare administration, or a related field.
  • At least two years of experience working with HCBS programs or similar community-based service programs.
  • Strong knowledge of federal and state HCBS regulations and compliance requirements.
  • Excellent organizational and communication skills, both written and verbal.
  • Proficiency in data management and reporting tools.

Preferred Qualifications:

  • Master’s degree in social work, public health, healthcare administration, or related discipline.
  • Experience working with diverse populations including elderly and individuals with disabilities.
  • Familiarity with Medicaid waiver programs and managed care systems.
  • Certification in case management or program coordination.
  • Experience with quality improvement methodologies and project management.

Responsibilities:

  • Coordinate and manage daily operations of HCBS programs, ensuring adherence to all applicable regulations and standards.
  • Collaborate with service providers, clients, families, and community partners to develop individualized care plans and resolve service delivery issues.
  • Monitor program performance through data collection, analysis, and reporting to identify trends and areas for improvement.
  • Conduct training and provide technical assistance to staff and providers to maintain high-quality service delivery.
  • Serve as a liaison between regulatory bodies and program participants to ensure compliance and address any concerns or audits.
