Position: Targeted Case Manager

Full-Time | Monday–Friday

Pay: $20.00–$24.00 per hour (based on experience)

$1,500 Sign-On Bonus

  • $750 after 90 days of employment
  • $750 after one year of service

About the Position

The Targeted Case Manager plays a vital role in helping guests transition from homelessness to stable, permanent housing. Working within our low-barrier shelter, you'll partner with individuals experiencing complex challenges—including mental illness, substance use disorders, trauma, and chronic homelessness—to develop individualized housing plans and connect them with the resources they need to succeed.

This position provides supportive housing services and will deliver Section 13 and/or Section 17 MaineCare Targeted Case Management services for eligible participants.

You'll work alongside an experienced multidisciplinary team committed to reducing barriers, promoting recovery, and helping people achieve long-term housing stability.

What You'll Do

  • Develop individualized service and housing plans that reflect each guest's strengths, goals, and needs.
  • Provide comprehensive case management using Housing First, trauma-informed, and harm reduction principles.
  • Deliver Section 13 and/or Section 17 Targeted Case Management services for eligible participants.
  • Assist guests in obtaining permanent housing, income, benefits, healthcare, behavioral health services, transportation, employment resources, and other community supports.
  • Advocate for guests while building collaborative relationships with landlords, housing providers, healthcare organizations, and community partners.
  • Support guests as they transition into housing and provide follow-up services to promote long-term housing retention.
  • Maintain timely and accurate documentation in HMIS, electronic health records, and other required systems.
  • Participate as an active member of a collaborative team dedicated to ending homelessness in Central Maine.

Qualifications

Required

  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Behavioral Health, or a related field or current MHRT-C certification.
  • Valid driver's license and reliable transportation.
  • Strong communication, organizational, and documentation skills.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.

Preferred

  • Experience working with individuals experiencing homelessness, mental illness, substance use disorders, or complex trauma.
  • Knowledge of Housing First, trauma-informed care, motivational interviewing, or harm reduction practices.
  • Familiarity with MaineCare Targeted Case Management, HMIS, or electronic health records.
