Position:  Children’s Care Manager Fee for Service 

Service Unit Compensation: $40.00 per service unit, or $42.00 per service unit for Bilingual Care Managers

Training Compensation: $30.00 per hour for all required training

Hours: Up to 20 hours per week, with day, evening and weekend availability 

Location: Travel throughout Suffolk County (within defined catchment area)

Start Date: Immediate



The Association for Mental Health and Wellness is seeking a passionate Fee for Service Children’s Care Manager (CCM) to make a difference in lives of children and their families. This position helps families of children living with complex behavioral and/or mental health conditions to navigate complex systems of care; building supportive relationships and developing individualized care plans and will also be responsible for completing the enrollment process, developing a care plan and maintaining detailed accurate case notes, reports and documentation. The fee for service CCM total compensation per service unit includes travel time, required administrative tasks, and the anticipated time to complete the client services. Service delivery is community based throughout Suffolk County. 


 

Key responsibilities will include, but are not limited to:

  • Meeting with children and families in-person providing comprehensive care management services; including initial consent, enrollment, eligibility processes, etc. 
  • Contact providers to obtain necessary eligibility documentation
  • Maintaining detailed, accurate and timely case notes and documentation in the EHR
  • Carrying a mixed caseload of clients in outreach and enrolled cases. 
  • Providing comprehensive Care Management, assessments, linkages and referrals. 


Qualifications:

  • A Bachelor’s degree in a human service/health discipline and two years’ experience working with children is required. Candidates with Master’s degrees in same discipline are also encouraged to apply.
  • A minimum of 2 years of relevant experience in a human services field, such as child welfare, mental health, or special education
  • Attention to detail to ensure accuracy in the consent and enrollment process.
  • Excellent inter-personal communication including the ability to establish empathic, empowering relationships with children and families who have experienced behavioral health issues, substance use, complex trauma as well as physical conditions which impact their functioning.
  • Strong collaborative communication skills with health/mental health professionals, educators, child welfare services and involved in the care.
  • Current or previous certification in completing Child and Adolescent Needs and Strengths Assessments (CANS) a plus.
  • Experience with juvenile justice system a plus
  • Understanding of the key elements of recovery and wellness, including the role of family and self-help;
  • Ability to prioritize work for efficient and quality care 
  • Basic knowledge of the children’s public health/mental health system, child welfare, public entitlement and social service programs;
  • A reliable self-starter with the ability to work both independently and as part of an interdisciplinary team. Candidate must be flexible in availability to work evenings, after school, weekends in order to meet with employed caregivers.
  • Strong writing and computer skills, as electronic health record is used for documentation. Basic knowledge in computer applications such as Microsoft Word, Excel, Outlook and EHR.
  • Clean and Valid NYS Driver’s License.
  • Insured, reliable NYS registered passenger vehicle.
