Summary 

The bilingual Spanish family Coordinator for the School-based Early Support Program plays a pivotal role in providing comprehensive support and intervention services to children and families within the school setting. This position requires a compassionate and organized individual with a strong understanding of child development, family dynamics, and community resources. The Family Coordinator will work closely with school staff, families, and community partners to assess needs, develop individualized plans, and coordinate services to ensure the well-being and academic success of children.

Essential Functions: 

 

  1. Assessment and Planning: 

  • Conduct thorough assessments of children and families to identify strengths, needs, and goals. 

  • Collaborate with school personnel and other stakeholders to gather relevant information and develop comprehensive support plans. 

  • Utilize evidence-based practices and assessment tools to inform decision-making and goal-setting. 

 

  1. Service Coordination: 

  • Coordinate and facilitate access to a range of support services, including educational, mental health, medical, and social services. 

  • Act as a liaison between the school, families, and external service providers to ensure seamless communication and coordination of care. 

  • Monitor service delivery and progress towards goals, adjusting plans as needed to address evolving needs. 

 

  1. Advocacy and Empowerment: 

  • Advocate for the needs and rights of children and families within the school and larger community. 

  • Empower families to navigate systems and access resources by providing information, education, and support. 

  • Promote family engagement in decision-making processes and encourage self-advocacy and resilience. 

 

  1. Documentation and Reporting: 

  • Maintain accurate and up-to-date case records, progress notes, and documentation in accordance with program requirements and confidentiality standards. 

  • Prepare and submit timely reports to document outcomes, achievements, and challenges encountered in service delivery. 

  • Ensure compliance with all relevant policies, procedures, and regulations governing case management and confidentiality. 

 

  1. Collaboration and Professional Development: 

  • Collaborate with interdisciplinary teams, including school personnel, social workers, counselors, and community agencies, to enhance support services and achieve positive outcomes. 

  • Participate in ongoing training and professional development activities to enhance knowledge and skills related to child development, family systems, trauma-informed care, and cultural competency. 

  • Engage in reflective practice and seek supervision and support as needed to promote personal and professional growth. 

  • Performs other related duties as assigned.

  • None 

 

Minimum Qualifications 

  • Bachelor’s Degree in Social Work, Psychology or Social Services.

  • Must be bilingual Spanish/English  

  • At least 2 years’ experience in related field. 

  • Good interpersonal skills, ability to work with diverse social, economic, and cultural groups.  

  • Proficiency in Microsoft Office. 

  • Ability to plan and carry out assignments independently.  

  • Ability to prioritize, adhere to timelines and multi-task.
