The Just One ProjectAbout Us
The Just One Project's mission is to increase access to food, resources, and supportive services to build a stronger, healthier, and more connected community. The Just One Project has a culture of attracting the most passionate and talented people to represent The Just One Project by reinforcing strong values, work life balance, and diversity. We know that our company culture ultimately affects client experience. Every program is designed with our clients in mind; we are "client centric."
Everything we do is guided by our core values: Respect, Integrity, Innovation, and Compassion, and by our Gold Standard of service. We believe every person who walks through our doors deserves to be treated with the same care, dignity, and respect as you'd want for your own family.
That commitment comes to life through the direct support we provide to individuals and families in our community. Our Community Market Case Managers help clients navigate challenges, connect with essential resources, and develop meaningful goals toward greater stability and self reliance. We're looking for someone who can build strong relationships with clients while bringing accuracy, organization, and accountability to every part of their work.
About the Role
As a Community Market Case Manager, you'll provide comprehensive case management services to older adults who may need assistance navigating food access, benefits, housing, healthcare resources, financial stability, transportation, and other supportive services.
You'll work directly with clients to assess their needs, develop individualized service plans, connect them with appropriate community resources, coordinate referrals, and provide ongoing support as they work toward greater stability, independence, and quality of life.
This is a highly client facing and data driven role. You'll balance meaningful direct service with detailed case documentation, including maintaining accurate client records, tracking services and outcomes, and completing required documentation in HMIS and Salesforce within established timelines.
