POSITION SUMMARY
The Family Shelter Care Manager provides fast-paced care management services to families residing in the family shelter. The Care Manager is responsible for constantly engaging families to access housing, employment, income, health, transportation, childcare, documentation, and other service needs.
The Care Manager is responsible for maintaining accurate and timely client records, including HMIS documentation, service plans, case notes, assessments, referrals, and housing-related documentation.
REQUIRED QUALIFICATIONS
- Bachelor’s degree in social work.
- Minimum of two years of experience in human services, case management, care management, housing services, homeless services, or a related field.
- Experience working with families in a fast-paced environment.
- Knowledge of community resources and social service systems.
- Experience with crisis intervention and de-escalation.
- Ability to maintain accurate and timely documentation, detailed case management notes, and maintain confidentiality.
- Ability to work effectively with individuals from diverse backgrounds.
REQUIRED SKILLS
- Ability to communicate effectively in writing and verbally while providing exceptional customer service.
- Excellent organization and time-management skills with the ability to manage multiple priorities and a caseload.
- Strong documentation and recordkeeping skills.
- Ability to conduct effective care management sessions and housing-focused assessments.
- Ability to work independently while functioning effectively as part of a multidisciplinary team.
- Ability to exercise sound judgment and maintain professionalism in challenging situations.
ADDITIONAL REQUIREMENTS
- Valid Florida driver's license and reliable transportation.
- Ability to travel locally for home visits, community meetings, appointments, and other program-related activities.
- Flexibility to participate in occasional evening or weekend activities as required.
- First Aid and CPR certification within 30 days of employment, if not currently certified.
JOB TYPE: Full time (position will require local travel and occasional work hours outside of regular M-F work week).
PAY RANGE: $44,000 Annually
