Job Summary:
Join our compassionate and collaborative team as an ER Enhancement Liaison, where you will play a vital role in providing advanced substance use and mental health treatment to individuals in need. You will engage with consumers, conduct assessments, and connect them to essential resources, all while working under the guidance of experienced leadership. In this role, you will serve as the first point of contact for referrals, conduct community outreach, and provide crisis intervention as needed. Your efforts will contribute to the stabilization and recovery of individuals facing challenges in their lives.

Key Responsibilities:

  • Collaborate with the ERE Coordinator to serve as the first point of contact for ERE referrals from Behavioral Health Response and the system’s sobering center.
  • Conduct community outreach to engage consumers referred to ERE and perform initial assessments of their needs.
  • Establish rapport with consumers, facilitating discussions about their strengths, needs, and preferences to motivate recovery.
  • Provide crisis intervention and perform preliminary linkage and coordination of services for individuals seeking treatment.
  • Offer short-term case management and maintain regular contact with consumers and service providers to monitor service delivery.
  • Report on individual outcomes at 30-day, 3-month, and 6-month intervals.
  • Maintain accurate electronic records and documentation as required by the Department of Mental Health.
  • Collaborate with the ERE Outreach Team and participate in regular meetings and training sessions.
  • Build and maintain relationships with hospital staff and community resources to ensure appropriate referral patterns.

Education, Experience, and/or Credential Qualifications:

  • Be at least 21 years of age.
  • Minimum of six months related experience in the Behavioral Health Field, including case management and outreach.
  • Graduate from an accredited college or university with a Bachelor’s Degree in social work, counseling, psychology, or a closely related field.
    OR
  • At least two years of relevant, qualifying full-time equivalent experience in human service delivery, demonstrating skills in developing positive client relationships and navigating complex service systems.
    OR
  • Possess the Certified Peer Specialist credential.

Additional Qualifications:

  • Successful completion of background checks, including criminal record, driving record, abuse/neglect, and fingerprint checks.
  • Current driver’s license, acceptable driving record, and current auto insurance.
  • Strong verbal and written communication skills, along with computer and organizational skills.
  • Must be comfortable meeting with consumers in the community and transporting them in a personal vehicle.

Physical Requirements:

  • ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull, or otherwise move objects, including the human body.
  • Sedentary work involves sitting most of the time but may involve walking or standing for brief periods.
