Perimeter Behavioral Hospital of Arlington is seeking a compassionate and motivated Discharge Planner to join our team. This is an excellent opportunity to play a vital role in supporting patients' successful transitions from inpatient treatment to ongoing care and recovery.

Position Summary

The Discharge Planner is responsible for coordinating and facilitating discharge planning activities to ensure continuity of care following hospitalization. This role collaborates closely with patients, families, treatment team members, healthcare providers, insurance companies, and community resources to develop and implement effective aftercare plans that support positive patient outcomes.

Essential Responsibilities

  • Participate as an active member of the multidisciplinary treatment team, including treatment planning meetings, staff meetings, and assigned committees.
  • Collaborate with therapists, providers, utilization review staff, and insurance companies to coordinate discharge planning activities and required documentation.
  • Assist with medical record completion and chart auditing activities.
  • Assess patient needs and work with the treatment team to ensure appropriate services and resources are in place.
  • Promote family involvement in the treatment and discharge planning process.
  • Coordinate communication with referral sources, mental health professionals, and community agencies to develop comprehensive aftercare plans.
  • Arrange discharge services and resources to support patient success following hospitalization.
  • Maintain accurate and timely documentation in accordance with facility standards and regulatory requirements.

Qualifications

Education

  • Bachelor's degree in social work, Psychology, Counseling, or a related behavioral health field preferred.

Experience

  • Minimum of one (1) year of experience in an inpatient behavioral health setting or a comparable healthcare environment strongly preferred.
  • Knowledge of clinical treatment principles, case management, and discharge planning processes is highly desirable.

Additional Requirements

  • Current Basic Life Support (BLS) certification.
  • De-escalation Training: Handle with Care (HWC) certification.
  • Ability to work flexible schedules and overtime as needed.

Schedule

  • Full-Time
  • Monday through Friday
  • 8:00 AM to 4:30 PM
