Description

The Disaster Case Manager provides comprehensive, client-centered case management services to individuals and families affected by disasters. Working under general supervision, this position assesses disaster-related unmet needs, develops individualized recovery plans, coordinates services, and connects clients with community resources to support long-term recovery. The Disaster Case Manager serves as an advocate, helping survivors navigate available assistance while promoting self-sufficiency and sustainable recovery.

This is a temporary grant-funded position expected to end on or before August 2027.

Essential Duties and Responsibilities 

· In either an office or a field setting, interviews clients about recovery needs Conduct comprehensive assessments of disaster-affected individuals and families in office, home, or community settings to identify disaster-related unmet needs.

· Conduct outreach activities to identify households that may be eligible for grant-funded recovery programs.

· Collect and verify demographic, financial, and other required documentation to determine program eligibility.

· Assist clients with gathering and submitting documentation required for grant-funded recovery assistance.

· Develop, implement, and monitor individualized disaster recovery plans that promote safe, sanitary, secure, and sustainable housing.

· Assist clients in identifying disaster-related recovery priorities while connecting them with appropriate resources for non-disaster needs.

· Coordinate referrals to community organizations, government agencies, and other service providers to address unmet needs.

· Maintain accurate, timely, and complete client records in accordance with program requirements, agency policies, and confidentiality standards.

· Document services, prepare reports, and conduct regular follow-up to evaluate client progress toward recovery goals.

· Collaborate with partner agencies, Long-Term Recovery Groups (LTRGs), Unmet Needs Committees, and other disaster recovery stakeholders.

· Participate in community outreach, education, and recovery coordination activities as assigned.

· Provide services in a culturally responsive, trauma-informed, and client-centered manner.

· Maintain professional boundaries while advocating effectively for client recovery needs.

· Perform other duties as assigned.

Supervisory Responsibilities:

This position has no supervisory responsibilities.

Minimum Qualifications:

  • High school diploma or GED      required; Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
  • Minimum of two (2) years of experience in case management, human services, disaster recovery, or a  related field.
  • Experience conducting field-based work with minimal supervision.
  • Valid driver’s license, reliable transportation, and an acceptable driving record.

Knowledge, Skills and Abilities:

  • Knowledge of case management principles and strengths-based service delivery.
  • Strong interviewing, assessment, and active listening skills.
  • Excellent verbal, written, and interpersonal communication skills.
  • Ability to establish rapport with individuals from diverse cultural and socioeconomic backgrounds.
  • Strong organizational and time management skills with the ability to manage multiple cases      simultaneously.
  • Ability to work independently while exercising sound judgment and maintaining confidentiality.
  • Proficiency in Microsoft Office (Word, Excel, Outlook) and electronic case management systems.
  • Ability to collect, analyze, and document information accurately.
  • Strong problem-solving, conflict resolution, and advocacy skills.
  • Ability to work collaboratively with community partners and multidisciplinary teams.
  • Ability to adapt to changing priorities and work effectively in disaster recovery environments.
  • Bilingual proficiency is preferred.

Physical Requirements:

This position requires a combination of office and field work.

  • Frequent sitting, standing, walking, speaking, hearing, and computer use.
  • Ability to conduct home visits and travel throughout assigned service areas.
  • Occasional lifting of up to 20 pounds.
  • Ability to travel by personal vehicle or commercial transportation as needed.

Working Conditions:

  • Standard office environment with frequent travel to client homes and community locations.
  • May require evening or weekend work based on disaster response and recovery needs.
  • Work may involve exposure to  disaster-affected environments, including damaged homes and neighborhoods.
  • Requires the ability to respond appropriately in crisis situations while maintaining professionalism and compassion.
