Position Summary

We are seeking a compassionate, organized, and experienced Director of Social Services to lead and oversee social service operations within our long-term care facility. The ideal candidate will advocate for resident well-being, support families through care transitions, and ensure compliance with all social service regulations and standards.

This position plays a vital role in enhancing residents’ quality of life through assessment, counseling, care planning, and coordination of services.

Key Responsibilities

  • Conduct psychosocial assessments and update care plans based on resident needs
  • Provide counseling and supportive services to residents and families
  • Assist with admission, discharge, and transfer processes, ensuring appropriate communication with physicians and families
  • Facilitate discharge planning, including coordination with community resources and services
  • Advocate for resident rights and provide education to staff on psychosocial issues
  • Document social services interventions and progress in resident records
  • Participate in interdisciplinary care conferences and survey preparation
  • Supervise and support social services staff as applicable
  • Ensure compliance with federal, state, and facility regulations related to social services

Qualifications

Required

  • Bachelor’s degree in Social Work required
  • Licensed Social Worker in the state of Michigan

Preferred

  • Experience in long-term care or healthcare social services
  • Leadership or supervisory experience in a healthcare setting

Physical & Work Requirements

  • Ability to move freely throughout the facility and interact with residents and families
  • Occasional lifting of up to 40 lbs and pushing wheelchairs as needed
