Pay Range: $62k-$84k annually based on previous working experience. 

Schedule: Fulltime, occasional weekends.

What You Bring (Requirements)

  • Bachelor’s Degree in Social Work or related degree in social sciences; licensed where applicable by the state; state requirements may dictate supervision and degree type.
  • Minimum of one year of social work experience, preferably in a long-term healthcare setting; knowledge of long-term healthcare and public health settings; familiarity with Federal and State regulations and requirements for long-term care.
  • Ability to read, speak and write fluently in English; excellent communication and phone skills; ability to prepare written assessments and correspondence; ability to read and interpret rules and regulations.
  • Ability and willingness to work flexible hours, such as during evenings and weekends, for planned activities and special events, family meetings and conferences, and as necessary, to facilitate admission calls, admissions and tours. Manages “on-call” schedule with other social work staff by pre-arrangement to cover facility for weekends/evenings admissions per Executive Director.

What You’ll Be Responsible For

  • Leads and directs Social Service Department.
  • Manages and directs activities of department in accordance with Volunteers of America National Services’ policies and procedures and guidelines.
  • Coordinates resident admissions.
  • Coordinates and leads admission transfer and discharge team.
  • Completes assessments of residents as necessary for care planning, MDS, RAPS, PPS and other required documents.
  • Initiate and/or participate in multidisciplinary care conferences held for residents, as appropriate to state and federal guidelines.
  • Performs other duties or special assignments as directed by the Executive Director.
