We are excited to be on the search for our next Social Services Director to join our exceptional leadership team! Our Skilled Nursing Facility is located in Long Beach, CA with a proven track record of high-quality care to our residents. 

The ideal candidate has experience as a Supervisor or Director of Social Services in a Skilled Nursing Facility or Long-Term Care Facility for a minimum of 1 year. Our clinical and non-clinical teams are regarded as amongst the best in the area providing 5-Star customer service.

Pay Range: $70,304-78,000/annually

Job Overview: 

As the Social Services Director, you will be responsible for planning, developing, organizing, implementing, evaluating, and directing social service initiatives, ensuring the medically related emotional and social needs of each resident are met and maintained on an individual basis.

  • Lead and assist in the planning, development, organization, implementation, evaluation, and direction of the facility's social service programs.
  • Actively participate in discharge planning, and the development and implementation of social care plans and resident assessments.
  • Conduct interviews with residents and their families as needed.
  • Maintain accurate and timely Social Service progress notes, documenting responses to treatment plans and adjustments to institutional life.
  • Facilitate access to community social, health, and welfare agency resources to meet resident needs.
  • Maintain consistent contact with residents' families providing progress reports as necessary.
  • Assist in the effective resolution of grievances voiced by residents, families, responsible parties, or ombudsman.

Qualifications:

  • Preferably a Bachelor’s or Master's in Social Work or related field (Sociology, Special Education, Rehabilitation Counseling, Psychology, etc.), but not required 
  • Minimum of 1 year experience as a Supervisor in a Skilled Nursing Facility
