Are you passionate about making a meaningful difference in the lives of residents and their families? We are looking for a compassionate, collaborative, and dedicated Director of Social Services to join our leadership team!

In this important role, you’ll lead our Social Services Department while helping ensure the emotional and social needs of our residents are recognized, supported, and met. You’ll work closely with residents, families, interdisciplinary team members, and community resources to provide individualized support throughout each resident’s journey.

What You’ll Do

As our Director of Social Services, you will:

• Lead and oversee the day-to-day operations of the Social Services Department.
• Develop individualized social service care plans in partnership with residents and families.
• Participate in resident care planning and help ensure social and emotional needs are addressed.
• Coordinate discharge planning and connect residents with appropriate community and post-discharge resources.
• Help residents and families navigate Medicare, Medicaid, financial assistance programs, and available resources.
• Coordinate services such as counseling, pastoral care, and legal assistance when needed.
• Collaborate with other departments to promote continuity of care and a positive resident experience.
• Recruit, supervise, develop, and support Social Services team members.
• Maintain required documentation and ensure compliance with applicable federal, state, and local requirements.

What We’re Looking For

We’re looking for someone who brings both professional knowledge and genuine compassion to the role.

Qualifications include:

• Bachelor’s degree in Social Work or Social Services Designee 
• Licensure where required by state regulations.
• At least one year of supervised social work experience in a healthcare setting working directly with individuals.
• Strong interpersonal, communication, and counseling skills.
• Knowledge of Medicare/Medicaid processes and long-term care regulations.
• Ability to manage multiple priorities while maintaining accurate and timely documentation.
• A collaborative approach and commitment to treating residents and families with dignity and respect.
