The Director of Senior Services is a unique leadership opportunity to build and lead a community-based senior services program from the ground up. This individual will oversee the program's strategy, operations, partnerships, financial performance, and outcomes while serving as the primary ambassador to community organizations, faith-based partners, healthcare providers, and senior-serving networks.

Minimum Qualifications

  • Bachelor's degree in Healthcare Administration, Public Health, Nursing, Social Work, Gerontology, Business Administration, or a related human services field.
  • Minimum five (5) years of progressively responsible management experience in healthcare, human services, community programs, or related operations.
  • Demonstrated experience developing and launching a program, service line, or community initiative from concept to implementation.
  • Proven success developing, managing, and sustaining community partnerships.
  • Experience developing and managing operational budgets.
  • Strong leadership, project management, and strategic planning skills.

Preferred Qualifications

  • Master's degree in Public Health (MPH), Healthcare Administration (MHA), Social Work (MSW), Business Administration (MBA), Gerontology, or a related field.
  • Experience in a Federally Qualified Health Center (FQHC) or other safety-net healthcare setting.
  • Experience managing senior services, adult day health programs, aging services, or community-based healthcare initiatives.
  • Established relationships with organizations serving older adults within the SJCH service area.
  • Experience with grant writing, grant management, and program development.
  • Bilingual English/Spanish skills highly desirable.

Responsibilities


Program Leadership & Strategy

  • Design, develop, launch, and continuously improve the Senior Services Program.
  • Develop the program's service model, participant eligibility criteria, operational workflows, staffing plans, and service offerings.
  • Establish policies, procedures, and standards for participant enrollment, service delivery, documentation, and care coordination.
  • Define program outcomes, performance metrics, and evaluation strategies.
  • Lead phased implementation plans, pilot programs, and expansion initiatives.
  • Monitor program growth and patient engagement while identifying opportunities for service enhancement.

Community Partnership Development

  • Build, cultivate, and maintain strategic partnerships with community organizations, faith-based groups, senior centers, healthcare organizations, and local agencies.
  • Negotiate partnership agreements and establish referral pathways that support program objectives.
  • Represent SJCH within community coalitions, aging networks, and senior service organizations.
  • Serve as the primary spokesperson and relationship leader for the Senior Services Program.

Clinical & Operational Coordination

  • Collaborate with SJCH clinical leadership to provide community-based preventive health programs, screenings, immunizations, and wellness initiatives.
  • Develop referral pathways connecting participants to primary care, behavioral health, dental, specialty care, and supportive services.
  • Ensure all services meet organizational standards for quality, privacy, documentation, and compliance.
  • Oversee adherence to HIPAA requirements and all applicable regulatory standards.

Leadership & Team Development

  • Recruit, hire, onboard, supervise, mentor, and develop program staff and volunteers.
  • Establish performance expectations and provide ongoing coaching and professional development.
  • Foster a culture centered on dignity, respect, diversity, inclusion, and person-centered care.
  • Ensure staff and volunteers complete required training and maintain program competencies.

Financial Management & Compliance

  • Develop and manage program budgets in partnership with Finance.
  • Monitor expenditures, track performance metrics, and analyze variances.
  • Collaborate with Grants and Development teams to identify and pursue funding opportunities.
  • Ensure compliance with federal, state, local, and organizational requirements governing senior services.
  • Provide regular reports and updates to executive leadership regarding program outcomes, enrollment, participation, financial performance, and community impact.
