Director of Health Homes and Special Projects

Pay Grade: 11

Under the general supervision of the Medical Director, the Director of Health Homes and Special Projects is responsible for the planning, growth, overall administration, and oversight of Medicaid Health Home programs at DWIHN, and project management of special project initiatives which fall outside standard departmental workflows. Health Home activities include the Behavioral Health Home (BHH) and SUD Health Home (SUDHH). This Director’s “Special Projects” function coordinates teams and resources to ensure strategic projects are delivered on time in alignment with organizational goals. This role includes supervision of staff and the development and implementation of program policies and procedures.


PRINCIPAL DUTIES AND RESPONSIBILITIES:

 

  • Oversees the overall design, ongoing implementation, and oversight of the Behavioral 

Health Home and SUD Health Home programs.

  • Serves as a regional liaison between DWIHN’s Health Home Partners and MDHHS staff/contractors.
  • Ensures Health Home staff coordinates with Health Home provider partners to achieve performance outcomes.
  • Works as part of the interdisciplinary team to facilitate consultation, collaboration, and care continuity across care settings for the best possible treatment.
  • Identifies opportunities for program growth and performance improvement, and acts upon those identified gaps by creating an action plan and leading team members to carry it out. 
  • Coordinates and utilizes Health Information Technology (HIT) with the Health Home 

team to maximize care coordination and care management.

  • Champions practice transformation based on health home principles and emerging best practices.
  • Collects and reports on data that permits an evaluation of increased coordination of care 

and chronic disease management.

  • Monitors Health Home performance and leads quality improvement efforts.
  • Provides oversight to all training, onboarding, and technical assistance across systems for Health Home initiatives. 
  • Convenes collaborative workgroups to address implementation issues and/or barriers.
  • Analyzes the service delivery system and makes recommendations to DWIHN regarding 

service needs.

  • Reviews funding needs in support of services in consultation with the Vice-President of 

Finance and makes recommendations for Board Actions.

  • Ensures compliance with Medicaid Health Home models. 
  • Manages the evaluation and selection of providers who meet the certification standards 

for Health Homes.

  • Works with local and regional Universities and their staff to support Research and 

Evaluation activities aligned with Health Home Initiatives.

  • Creates presentations for state and national meetings related to Health Home initiatives.
  • Stays informed and up to date on Health Home programs.
  • Reviews proposed special projects with careful consideration regarding the urgency and impact of the issues/changes to be resolved by the project. 
  • Contacts stakeholders and consults with senior leadership to gain additional background information and context on proposed projects to inform prioritization and planning. 
  • Prepares and presents recommendations on reviewed projects, including a recommendation for adoption or deferral, rationale, recommended project priority level, and resource allocation to the immediate Supervisor. 
  • In partnership with their direct Supervisor, makes dispositions on submitted projects and determines priority level ranking among other special projects. 
  • Creates project plans and tracks deadlines, project team members, assignments, and deliverables. Defines project scope, identifies stakeholders, and assembles the project a team, and establishes the tracking and communication cadence for the project duration. 
  • Direct and manage project team members to deliver high-quality results.
  • Report on project status and identify barriers to deliverable completion. Bring together key stakeholders to swiftly resolve identified barriers. 
  • Works collaboratively with Directors of the units within DWIHN to meet the overall goals 
  • and objectives of DWIHN.
  • Supervises program and support staff as assigned.
  • Performs related duties as assigned.

REQUIRED EDUCATION:

A Bachelor’s Degree from a recognized college or university in the Human Services, the Social Services, Nursing, Public Health, Public Administration, Healthcare Administration, Health Management, Psychology, Counseling, Social Work or a related field.

 

REQUIRED EXPERIENCE:

Two (2) years of professional experience in a behavioral healthcare or mental health setting.

 

AND

 

One (1) year of professional experience working in community-based programs with agencies that provide services for persons with behavioral health disorders. 

 

REQUIRED LICENSE(S).

A valid State of Michigan Driver’s License with a safe and acceptable driving record.

 

WORKING CONDITIONS:

Work is usually performed in an office setting but requires the employee to drive to different sites throughout Wayne County and the State of Michigan. This position can work remotely with supervisory approval.
