Director of Clinical Risk Management

The Director of Clinical Risk Management plays a pivotal leadership role within a fast‑paced, mission‑driven organization, guiding the agency’s clinical risk management program and strengthening systems that protect our clients, staff, mission, and assets.  

This position is responsible for identifying, evaluating, and mitigating clinical risks that may impact clients, staff, operations, or the organization. The Director of Clinical Risk Management oversees the review of clinical incidents, sentinel events, and high-risk clinical situations to promote a culture of safety, accountability, and continuous improvement.

POSITION SPECIFIC DUTIES & RESPONSIBILITIES:     

Clincal Risk Management Program  

• Direct the organization's clinical risk management program. 

• Develop and implement annual clinical risk management goals and objectives. 

• Identify emerging clinical risks and recommend mitigation strategies. 

• Collaborate with senior leadership to develop agency-wide risk reduction initiatives. 

• Provide guidance and consultation regarding clinical risk issues. 

• Support the organization's enterprise risk management framework. 

• Collaborate with Clinical Services to support clinical risk management training.  

Incident Review and Reporting 

• Lead clinical incident reviews, reporting, and follow-up and with coordination of efforts and documentation of incident reviews and reporting requirements for general, critical and sentinel incidents. 

• Lead or facilitate root cause analyses and corrective action planning. 

• Review and assist in development of plans for high-risk clients who threaten staff or facilities. 

• Continually evaluate and implement any changes or improvements to the incident report processes in the incident review reporting system.   

• Coordinate and implement any risk mitigation follow up or new agency initiatives derived from the Incident Review processes. 

• Partner with legal counsel on matters involving potential liability exposure. 

• Assist the Vice President of Legal & General Counsel in reporting risk trends to Risk Council, the agency’s executive leadership team, and PECQAR board committee. 

• Support the PECQAR committee through preparation of presentation materials and tracking all follow upon Create and monitor PBI reports for risk trends.  

• Collaborate with Compliance regarding reportable events and investigations. 

Agency Insurance Portfolio  

• Partner with the Director of Risk Management on the annual review and renewal of the agency’s insurance portfolio in collaboration with the General Counsel, agency’s insurance broker, and PECQAR.  

QUALIFICATIONS 

education: 

One of the following licenses required: 

• LCSW 

• LPC 

• LMFT 

EXPERIENCE: 

• Minimum five (5) years of progressively responsible clinical experience. 

• Minimum three (3) years of leadership, supervision, quality improvement, risk management, compliance, or clinical operations experience. 

• Experience conducting investigations, root cause analyses, or quality reviews preferred. 
   

CERTIFICATIONS/LICENSES: 

• Risk Management Certification Preferred 

• Must possess a valid Driver License and satisfactory driving record to use agency and/or personal automobile to travel to locations other than primary office.
