What are we looking for in a Crisis Outreach Professional?
Crisis Outreach Professionals are part of our Acute Care team, which provides intervention and support to individuals who are experiencing a mental health or substance use crisis. They take calls requesting crisis services and travel out in the community to provide crisis assessments, determine what level of treatment is needed to stabilize the situation, and make arrangements for hospital placements or outpatient treatment as appropriate. The goal is to stabilize the crisis using the least restrictive setting possible, while still ensuring the safety of the individual in crisis as well as the community. This role requires working closely with emergency responders, law enforcement, Comprehensive staff, and other community partners to coordinate care. The Crisis Outreach Professional position offers the opportunity to work both collaboratively and independently, have significantly varied workdays, and use creativity to resolve problems. This position is part of our Acute Care team serving Yakima County but will be based in our Sunnyside office and will provide outreach services throughout the Lower Valley. Regular daily travel is expected throughout the region from approximately Wapato to Benton City.

Crisis Outreach Professional duties may include:
MINIMUM EDUCATION & EXPERIENCE REQUIRED:
Master’s degree in social or behavioral science or related field. Must be eligible for designation as a Mental Health Professional and Designated Crisis Responder (2 years of experience for designation).
BEHAVIORAL HEALTH CONSULTATION:
Effectively provides consultation regarding behavioral health needs of clients.
Provides recommendations and interventions that are supportable by the medical team.
Facilitates coordination and communication between primary care and specialty behavioral health services.
Serves a liaison between law enforcement, Acute Care Services and outpatient teams.
Provides education on the topic of behavioral health and crisis intervention techniques as appropriate.
CHILDREN, YOUTH and YOUNG ADULT SERVICES:
Demonstrates and utilizes knowledge of development in children, youth and young adults.
Demonstrates and utilizes knowledge and understanding of behavioral health problems in children, youth and young adults.
Demonstrates and utilizes knowledge of and ability to work with issues of confidentiality, consent and capacity.
Demonstrates the ability to collaborate and coordinate with family, parents, caregivers and community and natural supports.
Communicates effectively with children and youth of differing ages and developmental levels
CRISIS EVALUATION/INTERVENTION:
Utilizes appropriate crisis intervention techniques for client stabilization.
Accurately screens for risk to self, others, property and the presence of grave disability and makes appropriate referrals.
Documents and reports and/or notifies as required by law.
Applies QPRT (Question, Persuade, Refer or Treat) principles in assessing risk and in safety and treatment planning.
Develops and documents appropriate safety plans.
CRISIS OUTREACH:
Makes initial contact with referent and client to gather information on presenting problem
Provides referrals and when necessary, arranges for appropriate follow up. Will provide follow up as directed by Crisis Outreach Professional (COP).
Responds to requests for outreach with Crisis Outreach Professionals to assist and for safety purposes
Assists team members by following up with clients admitted to Stabilization Bed to monitor symptoms and to arrange discharge.
Notifies treatment teams of any crisis contact with their assigned clients. Coordinates with treatment team for follow up contact and stabilization plan.
Answers Open Line/Crisis Line calls and screens for risk, makes appropriate referrals and staffs with COP. Reviews after-hours call center reports for the need to provide follow up contact or send to assigned treatment team.
CRISIS OUTREACH PROFESSIONAL EVALUATION:
Utilizes appropriate crisis intervention techniques for client stabilization.
Accurately assesses risk to self, others or property and the presence of a grave disability.
Formulates appropriate clinical decisions and makes appropriate referrals.
When needed, locates psychiatric hospital bed and ensures clients safety during the process.
Ensures pass down of case when bed cannot be found and arranges for on-site support if needed.
Outlines the plan for crisis stabilization when client is placed at Crisis Treatment Center or returned to natural supports.
Follows Acute Care Services Program Manual protocols for specific RCW’s and other Acute Care Services processes
GERIATRIC SERVICES:
Demonstrates and utilizes knowledge of normal aging process and age-related changes.
Demonstrates an understanding of neurocognitive disorders and their impacts on functioning.
Demonstrates awareness of how medical issues can affect mental status
SERVICE NOTES:
Completes all required service notes according to Comprehensive Healthcare’s guidelines, policies and procedures regarding format, content and timeliness.
Language used in service notes is respectful, clear, descriptive, concise and client-centered.
Service notes accurately depict treatment modalities and interventions provided within the clinician’s scope of practice and according to treatment plan.
Service notes accurately reflect and depict client status and progress consistent with assessment and reassessment
Qualifications:
Master's degree in social work, counseling, or related field
Driver's License and Vehicle Insurance
Previous experience providing behavioral health treatment is preferred
Must be eligible for designation as Mental Health Professional
Must be eligible for registration as Agency Affiliated Counselor
Must have valid driver's license and vehicle liability insurance
