The Counselor serves as a member of the Treatment team of WestCare Tennessee and works to ensure the delivery of quality services to their coworkers and participants within the program. This role provides counseling, assessment and treatment to eligible participants in suicide prevention and awareness.
 
Essential Job Functions:

Embraces and embodies the mission, vision, guiding principles, and goals of WestCare Tennessee.
Uphold strict confidentiality of participants information.
Collaborate with the Case Manager to provide services to participants.
Review Individual Service Plan (ISP) for participants.
Review the Wellness Recovery Action Plan (WRAP) with participants.
Develop individualized resident care plans.
Develop and review recovery treatment plans and assessments.
Score assessment instruments with guidance from the Project Director and Clinical Director.
Review service plans with the Case Manager.
Participate in clinical staffing decisions regarding the participants’ current strengths and resources and their need for additional resources.
Understand and implement a Dialectical Behavior Therapy (DBT) approach.
Implement a Collaborative Assessment and Management of Suicidality (CAMS) therapeutic framework for specific suicide assessments.
Facilitate Seeking Safety, Moral Reconation Therapy (MRT), and Motivational Interviewing (MI).
Facilitate as needed counseling sessions with eligible participants by either in person, via a virtual platform or telephone.
Maintain strong professional relationships with organizations and other community agencies to receive referrals and utilize office space for meetings and group activities.
Maintain strong professional relationship boundaries with participants served and organizations/ community agencies who refer individuals to WestCare Tennessee.
Assist in implementing a public health approach that blends community-based prevention with clinical strategies through community efforts.
Assist participants with establishing or re-establishing support systems within their community.
Provide support for the participants to improve access to the services needed.
Coordinates discharge and/or transfers to ensure appropriateness and continuity of care.
Complete Emergency Referral for continued treatment and care.
Engage and build rapport with participants to assist with tracking and follow-up.
Assist with meeting participants’ psychosocial needs.
Completes mandatory documentation within timelines established by applicable regulations.
Attend in-person and virtual program conferences and meetings, community meetings, and events as deemed necessary.
Attend necessary monthly integrated staff team meetings in coordinating the care and services for each participant including Veteran Family members.
Submit completed training and professional development training to the Project Director.
Submit weekly reports on activities to Project Director on Counseling outcomes.
Work closely with Clinical Director, Project Director, Case Manager, and Peer Support team regarding the collection of data and reporting of program outcomes.
QualificationsEssential Qualifications:
 
            Certifications/Licenses:

A valid driver’s license is required.
Licensed Clinical Social Worker (LCSW), Licensed Master of Social Work (LMSW), Licensed Professional Counselor (LPC-MHSP), Licensed Marriage Family Therapist (LMFT), or Licensed Clinical Pastoral Therapist (LCPT) required.


 

Education:


A Master's degree or higher in Counseling, Communication, Sociology, Criminal Justice, Social Work, Psychology, or a closely related human services field from an accredited institution is required for this position.


Experience and Competencies:


Experience in crisis triage.
Basic knowledge of suicide prevention services, veteran services, behavioral health, social services, judicial system, law enforcement or related industry.
Experience with counseling and administering assessments preferred.
Excellent communication skills including public speaking and group facilitation/training.
Some experience with performing outreach and marketing.
Ability to exercise good judgment and discretion.
Ability to work well in a team environment.
Ability to adhere to the highest standard of ethical conduct and confidentiality.
Professional appearance and demeanor.
Must understand rural populations who will be served.
Experience with using EHR systems.
Ability to work in different program systems in providing day-to-day operations.
Ability to stay highly organized and maintain time management.
Ability to utilize a computer and software that includes Microsoft 365.


 

Working Conditions:


Work is primarily performed in an office or indoor setting.
Some outdoor activities and travel will be required.
Some travel will be required for training and conferences.
Regular and prompt attendance is required throughout employment.
