Coordinator Discharge Planning- 40/hrs, Day Shifts (Temple Hospital)

The coordinator of DC planning is responsible for designing and coordinating comprehensive discharge plans in accordance with regulatory and patient satisfaction standards. Collaborates with nurse case managers and social workers regarding overall discharge planning needs. Below referenced licenses are only required for those individuals with education from LPN program or PTA program.

Education
Other Graduate of a licensed practical nurse program or Graduate of an accredited Physical Therapy Assistant Program or Bachelors of Social Work Degree Required

Experience
1 year experience with referral coordination Preferred
2 years experience in a Hospital Clinical setting Preferred

Licenses
PA Physical Therapist Asst Lic Required or
PA Practical Nurse License Required
