Job DetailsJob Location: DALLAS, TX 75201Position Type: Full TimePosition Summary: The Coordinated Outreach Expansion initiative provides funding for staff to conduct General and Encampment Outreach. Staff will be expected to provide both types of outreach throughout the geographical area of the Continuum of Care for the duration of the contract term. Outreach team assignments are made based on capacity needs, which fluctuate with the varying sizes of encampments to be closed.
Responsibilities:
General Outreach
Engaging and building relationships with neighbors experiencing unsheltered homelessness that effectively builds rapport and trust
Completing CAS Program Enrollment in HMIS
Assist neighbors in obtaining supporting documents to obtain vital documents, documents include but are not limited to: ID, Social Security card and Birth Certificates
Assist neighbors in obtaining Verification of Disability and letters of homelessness to verify chronic status
Submit necessary applications and payment requests for the vital documents
Provide neighbors with Stewpot IDs as needed
Service Coordination including referrals to services, shelter, connection to population-specific systems and/or to system-specific liaisons
The provision of services to meet basic needs including food and water, clothing and living materials, health and hygiene items, and income connection (if appropriate)
Pre-navigation activities including the collection of critical documents and warm hand-off to the housing case manager
Completing timely and accurate data entry and reporting including client program enrollments, client services, and uploading critical documents
Submit all Nexonia and time sheets in a timely manner
Other duties as assigned

Manage a monthly timesheet
Other duties as assigned
QualificationsQualifications
Bachelor’s degree in social work or social related field
1+ years working with people experiencing homelessness or other social service experience
