Summary

The Case Manager is a key member of the treatment team responsible for providing case management services to individuals with Level of Care 3 and/or 4 needs. This role conducts assessments, coordinates care, develops and monitors treatment plans, and connects clients with community resources to support stability and well-being. The position also provides individual and group case management services within school-based settings, supporting students across elementary, middle, and high school campuses. Local travel is required.

Compensation

$24.00-$28.00 an hour (1099)

Key Responsibilities


Conduct client assessments and identify service needs.
Develop, implement, and monitor individualized treatment plans in collaboration with therapists and other providers.
Provide case management services to clients with Level of Care 3 and/or 4 needs.
Facilitate coordination of care and connect clients with community resources and support services.
Complete timely and accurate documentation, including treatment plans, progress notes, and coordination of care records.
Provide individual and group case management services to students in school-based settings.
Facilitate scheduled group sessions and provide ongoing support to students on an assigned caseload.
Travel between assigned campuses and community locations as needed.
Participate in team meetings, trainings, and other required organizational activities.


Qualifications & Requirements


Master's degree in a Human Services field required.
Minimum of two (2) years of case management experience required.
Experience providing services to individuals with Level of Care 3 and/or 4 needs preferred.
CPR Certification required.
Knowledge of case management principles, healthcare management, and reimbursement procedures.
Experience developing treatment plans, coordinating care, and maintaining clinical documentation, including treatment plans and progress notes.
Experience working within behavioral health or mental health settings.
Ability to provide individual and group case management services in community and school-based settings.
Excellent organizational, time management, problem-solving, and multitasking skills.
Strong verbal and written communication skills.
Compassionate, professional, and client-focused approach to care.
Ability to work independently with minimal supervision.
Proficiency with computers, electronic documentation systems, and other technical resources.
Bilingual (English/Spanish) preferred.
Valid driver's license, reliable transportation, and ability to travel locally throughout assigned service areas and school campuses.
Must be eligible to work as an independent contractor (1099).
