Job Summary and Responsibilities

As an Outpatient Care Coordinator Social Worker, you will provide critical support and meticulous coordination, ensuring a seamless and integrated healthcare experience for patients navigating outpatient services.Every day you will expertly assess patient needs, facilitate appointments and referrals, educate patients on their care plans, and meticulously collaborate with multidisciplinary teams to optimize outcomes and enhance patient engagement.To be successful, you will demonstrate outstanding organizational skills, strong communication abilities, and an empathetic, proactive demeanor, contributing significantly to patient satisfaction and adherence to treatment regimens

  • Assists in the identification of member populations needing care coordination and social work intervention.
  • Acts as a liaison with patients and families to physicians, clinical staff, community resources and others.
  • Conducts a psychosocial assessment and acts as a patient’s advocate, by responding to and working with patients to address needs and resolve their concerns.
  • Provides Health Promotion activities and education for patients.
  • Functions as an active team member by effectively collaborating, communicating and coordinating among all responsible parties of an individual patient’s multidisciplinary health care team striving to eliminate fragmentation, duplication or gaps in health care.
  • Optimizes patient and family independence and self determination by providing education and health coaching, and assisting with improving problem solving and coping skills as needed.

Job Requirements

Required

  • Bachelor's in Social Work (BSW)
  • Certified Social Worker (CSW) in Nebraska

Preferred

  • 3-5 years of experience
