SUMMARY:
The Community Wellness Coordinator provides community based intervention, case management, and resource navigation services to Tribal members experiencing substance use or substance-related concerns. The Coordinator works directly with individuals to support engagement in treatment and recovery services, assist with transportation and access to care, address barriers to treatment, and provide ongoing follow-up and case management to support continued stability and wellness.
The Coordinator also leads the Tribe's substance use prevention, harm reduction, and community outreach efforts, with a particular focus on opioid-related prevention and response. Responsibilities include coordinating the Tribe's local opioid coalition, developing and implementing community prevention activities, supporting overdose prevention efforts, and developing partnerships with treatment, behavioral health, healthcare, and other service providers. The position may respond to community situations involving substance use in coordination with appropriate Tribal departments and assists individuals in connecting to appropriate services and supports.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Provide community-based intervention, outreach, and case management to Tribal members experiencing substance use or substance-related concerns.
- Engage members in a supportive and nonjudgmental manner and assist them in identifying appropriate treatment and supportive services.
- Connect members to substance use treatment, behavioral health, medical, recovery, and other community resources.
- Assist members with treatment referrals, appointments, intake processes, and other service connections.
- Provide transportation to treatment, appointments, and other appropriate services when authorized.
- Maintain regular contact with members receiving case management services and provide ongoing follow-up and support.
- Work with parents involved in dependency cases when substance use has contributed to the removal of their children, including supporting treatment engagement, service connections, and other goals related to reunification.
- Assist members in addressing barriers to treatment and stability, including transportation, housing, employment, benefits, and other supportive needs.
- Respond to community situations involving substance use in coordination with Tribal Police and other appropriate Tribal departments, providing supportive intervention, de-escalation, and connection to resources.
- Lead community-based substance use and opioid prevention, harm reduction, and education activities.
- Coordinate overdose prevention efforts, including Narcan education and distribution and other approved harm reduction activities.
- Serve as the staff lead for the Tribe's local opioid coalition and coordinate coalition meetings, activities, partnerships, and follow-up.
- Develop and maintain partnerships with treatment providers, healthcare providers, behavioral health programs, and other community organizations.
- Participate in community events, outreach, presentations, and prevention activities.
- Maintain appropriate documentation of member services, referrals, outreach, and program activities in the department’s case management system and assist with required reporting.
- Perform other related duties as assigned.
REQUIRED KNOWLEDGE & SKILLS:
- Knowledge of substance use, opioid misuse, treatment, harm reduction, recovery, and related community resources.
- Knowledge of case management, service navigation, crisis intervention, de-escalation, and community response practices.
- Knowledge of barriers individuals and families may experience when accessing treatment and supportive services.
- Knowledge of confidentiality and appropriate handling of sensitive member information.
- Skill in building trusting relationships and engaging individuals and families in a supportive, respectful, and culturally responsive manner.
- Skill in identifying needs, coordinating services, maintaining appropriate documentation, and communicating effectively with members and service providers.
- Ability to remain calm and exercise sound judgment when responding to challenging or crisis situations.
- Ability to work independently while effectively collaborating with Tribal departments, community partners, treatment providers, and other agencies.
- Proficiency in Microsoft Office programs, including Word, Excel, Outlook, PowerPoint, SharePoint, and OneDrive.
MINIMUM QUALIFICATIONS:
- Bachelor's degree in social work, behavioral health, human services, public health, or a related field; OR an equivalent combination of education, training, and relevant experience.
- Two (2) years of relevant experience in case management, substance use services, behavioral health, community outreach, social services, peer support, or a related field.
- Experience working directly with individuals and families experiencing substance use or other barriers to accessing services.
- Knowledge of substance use treatment, community resources, and service systems.
- Experience developing partnerships, coordinating community activities, or facilitating groups, coalitions, or community programs.
- Valid California driver's license and reliable transportation.
- Must successfully complete required background checks applicable to the position.
PHYSICAL REQUIREMENTS/WORKING ENVIRONMENT/SCHEDULE:
- This is a full-time position (Monday-Friday, 8:30AM to 5:00PM).
- This position requires working evenings and weekends as needed.
- Tasks are performed mostly indoors in an office environment; however regular field work will be required as part of case management efforts.
- May be required to push, pull, lift, and/or carry up to 40 pounds.
- The position involves standing, walking, bending, stopping, and long periods of sitting.
- Adequate hearing is required to hear/talk with all individuals and to use telephones consistently; work may require using a computer terminal for extended periods and some repetitive motion associated with preparing documents and using a computer mouse.
- Traveling may be required, including local, within the state, and national travel; work is performed in an office setting with some exposure to environmental stress; the noise level in the work environment is quiet to moderate with frequent communication with Tribal parents, Tribal members, Tribal attorneys, and the public.

BACKGROUND CLEARANCE REQUIREMENTS:
- Must successfully pass a comprehensive background investigation. This process includes mandatory fingerprinting.
- Must meet eligibility standards under the Cahuilla Band of Indians vehicle insurance requirements.
- Must complete and pass a pre-employment drug screening.
