About the Role

We are seeking a full-time Community Support Team (CST) Professional Team Lead to provide community-based therapy and case management services to adults with serious and persistent mental health challenges. This role focuses on empowering clients to enhance their ability to function and achieve their goals within the community.

Therapeutic and Clinical Services


Conduct comprehensive initial assessments and reassessments to evaluate client needs and strengths.
Provide individual and family therapy using evidence-based practices to support the mental health and well-being of clients.
Develop and implement person-centered plans to guide service delivery.
Monitor and manage symptoms through psycho-education and therapeutic interventions.
Facilitate client referrals to higher levels of care, such as ACT services, when appropriate.


Case Management and Advocacy 


Assist clients in accessing and coordinating benefits, resources, and services in the community.
Act as a liaison between clients, families, and community resources to ensure service continuity and effectiveness.
Educate clients and families on available community resources, benefits, and service options.
Provide ongoing support for rehabilitative and independent living skills development.


Community-Based Support 


Deliver services in off-site, community-based settings to foster client engagement and success.
Respond to client crises promptly and effectively, providing support and guidance as needed.
Documentation and Compliance 
Maintain accurate, timely, and compliant documentation, including service notes, treatment plans, and progress reports.
Monitor and evaluate the quality of services provided to ensure compliance with agency standards and client needs.

Compensation

This role is compensated on a salaried basis, providing consistent earnings aligned with the responsibilities and expectations of the position.


Compensation: $57,806.91 - $67,134


Placement within the range is based on licensure level, education, and experience.

Responsibilities


Provide individual, family, and/or group therapy sessions
Maintain accurate and timely clinical documentation
Collaborate with care teams as needed
Maintain compliance with all clinical and regulatory standards
Participate in supervision (if applicable based on licensure level)
 

QualificationsQualifications


A master’s degree from an accredited university in Counseling or a related field
Valid licensure to provide mental/behavioral healthcare in the state of North Carolina; Licenses include LCSW, LCSWA, LCMHC, LCMHCA, LMFT or Licensed Psychologist
A minimum of two years of experience working with individuals with mental and/or behavioral health issues that include writing person-centered plans, conducting effective group therapy and providing mental health diagnoses
General working knowledge of the human services delivery system in North Carolina
Valid driver’s license, auto insurance and reliable transportation
Employee supervisory experience preferred
Experience with Electronic Medical Records (EMR) preferred
