Community Outreach Representative – Home Care
Executive Home Care of Omaha is seeking a motivated, relationship-focused professional to build referral partnerships with skilled nursing facilities, rehabilitation centers, assisted living communities, and memory care facilities throughout Omaha.

Responsibilities
- Build relationships with social workers, discharge planners, and case managers.
- Visit healthcare facilities and introduce our in-home care services.
- Develop referral partnerships and follow up with prospective partners.
- Track outreach activities and referral opportunities.
Requirements
- 5+ years of experience in social work, discharge planning, or a related field.
- Knowledge of the Omaha healthcare and senior care community preferred.
- Strong communication and relationship-building skills.
- Self-motivated with reliable transportation.
