Acentra Health is looking for a Community Engagement Coordinator in CA to join our growing team.
Job Summary:
The Community Engagement Coordinator in CA is primarily responsible for building relationships with the local healthcare delivery system, focusing on integration efforts with community organizations and providers serving the Fee-for-service (FFS) population. This role is responsible for engaging with providers, community-based organizations, community health workers and other local healthcare services to support members experiencing transitions of care, prevent duplication of services, and maintain strong member–provider relationships.
Their collaboration with care coordinators and provider engagement specialists will contribute to the over overall effectiveness of supporting high risk member transitions and care delivery, while their expertise in community engagement, patient navigation and coordinating diverse beneficiary needs reflects the company's commitment to overall well-being.
***This is a remote position; however, candidates must reside in California. Candidates located in Northern California are preferred, including San Francisco, Santa Cruz, Sacramento, and the Central Valley. Bilingual candidates fluent in Spanish are also preferred. ***
Responsibilities:
- Field-based engagement specialist responsible for developing and enhancing community-based organization relationships and local community engagement strategies.
- Develop community engagement strategies that meet members “where they live” and establish a trusted environment for healthcare engagement.
- Interact and collaborate with a multidisciplinary team, including physicians, care coordinators, community health workers, case managers, pharmacists, and social workers/educators to ensure beneficiary needs and preferences for health services/information are shared.
- Educate members about community resources/options and advocate on behalf of the beneficiary.
- Establish a collaborative process working across the local health care delivery system to assist members in receiving timely access to care. This includes advocating and supporting members in identifying FFS treatment options and coordinating transitions of care in a timely manner to ensure quality outcomes. Maintain open communication with all appropriate parties.
- Foster a caring philosophy in all aspects of member transition process.
- Use independent judgment and discretion to address and proactively resolve barriers impeding member engagement.
- Maintain strict standards for client confidentiality and client-related information. Comply with all organizational, state, and federal regulations and policies on confidentiality.
- Prepare timely documentation, status updates, event notifications, and other documentation regarding beneficiaries in accordance with regulatory requirements and company policies and procedures.
- Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.

Qualifications
Required Qualifications
- Patient Navigator, CCM or Social Worker experience.
- 2+ years community healthcare engagement experience.
- Must have field-based healthcare experience focused on developing and strengthening community partnerships and implementing local community engagement strategies.
- Must reside in California.
- Strong communication (written/verbal), interpersonal, organizational, time management and communication skills.
- Demonstrated customer service skills.
- Must be willing to travel locally within California as needed.

Experience in Lieu of Degree
For non-clinical roles, or when not required by the contract specifically, the Company acknowledges that practical, hands-on experience can provide skills and competencies equivalent to formal education. As such, in cases where a Bachelor's degree may be required, the Company will accept a minimum of six (6) years of directly relevant professional experience in lieu of a degree. In instances where the candidate has an Associate's degree, the Company will accept a minimum of three (3) years of directly relevant professional experience in lieu of the Bachelor's degree.
