Program/Department Summary
The Community Health and Wellness sector provides services that support the physical, behavioral, and emotional health and wellbeing of individuals and communities. Our staff are certified Traditional Health Workers (such as Community Health Workers (CHW), Peer Support Specialists (PSS), and Doulas) who offer direct services such as healthcare and systems navigation, health insurance enrollment, physical and emotional health support, adjustment support, mental health resource navigation, and client-directed case management. We also engage in community outreach and physical and emotional health education via community workshops, psycho-education support groups, and by hosting monthly health clinics that provide dental services, health services, and community resource information.
Position Summary
The Community Health & Wellness Coordinator is responsible for coordinating the daily operations of Community Health and Wellness programs, including directly supervising a team of Community Health Workers (CHWs), Peer Support Specialists (PSS) and Birth Doulas from various background and experiences who work directly with clients, families, and healthcare personnel to support communities via healthcare navigation, health education, peer support, wellness groups, prevention education, individual client support, and other systems navigation.
The Community Health & Wellness Coordinator provides day-to-day supervision and mentoring of staff, ensuring trauma-informed, community-aware approach services reach community members. This position ensures staff have ongoing training and competence to provide referrals, direct services, and support to assist participants and provide accurate documentation of services as required. This position also ensures that Medicaid billing standards and documentation requirements are met.
This position works closely with the Community Health & Wellness Sector Manager and the Coordinators team, program funders, and community partners. The Community Health & Wellness Coordinator successfully coordinates, implements, reports, and evaluates the programs in their portfolio, as well as other projects as needed and assigned.
Essential Functions
- Possess and apply an impeccable degree of confidentiality and utilize knowledge of laws about confidentiality, disclosures, and releases of information to ensure program and staff compliance.
- With a fair degree of autonomy, coordinate implementation of CHW, PSS and Birth Doula programs, develop staff workplans, monitor caseloads and opening/closing of new cases, monitor expenses, monitor ongoing program eligibility (ORR and Medicaid eligibility), review and submit invoices, track deliverables and prepare reports and communicate with funders as needed.
- Supervise a team of CHWs, PSSs; and Birth Doulas. Hold regular case conferences and utilize problem-solving skills and sector expertise to support and grow staff; provide staff with backup during absences or difficult client situations.
- Implement a participant-centered, trauma-informed model to lead staff in assisting clients to attain their health and wellness goals.
- Ensure confidential participant records are recorded and maintained.
- Ensure proper service delivery documentation and case notes are completed to award or invoicing standards.
- Approve and process requests for client assistance or other award spending; ensuring all funder and IRCO requirements for award spending and billing are followed; work closely with IRCO’s Finance team on all financial matters.
- Onboard staff; lead performance evaluations in alignment with agency practices; ensure consistent interpretation of IRCO policies and internal guidelines; make recommendations to Community Health & Wellness Sector Manager on hiring and disciplinary actions when needed.
- Foster a supportive, positive, and accountable work environment.
- Regularly review participant files and service records, ensure data entry, case notes, updates, reports, timesheets and all other administrative tasks are completed accurately within given timelines.
- Periodically update protocols and procedures, with guidance from Community Health & Wellness Sector Manager and other teams.
- Research and share relevant training and professional development opportunities for staff. Support staff in maintaining Continuing Education Units (CEU) records and reporting to keep their credentials current.
- Build relationships with other IRCO programs and external service providers to ensure participants have access to appropriate, high-quality referrals.
- Participate in internal and external meetings as requested, with the goal of creating stronger connections for client services and advocating for client needs.
- Coordinate or conduct ongoing training of staff relevant to the Community Health & Wellness (CH&W) programs and their roles.
- As requested, prepare and conduct culturally and linguistically specific presentations on IRCO’s CH&W programs.
- Perform other duties and lead other projects as assigned.
Requirements
Education & Experience
MINIMUM JOB SPECIFIC QUALIFICATIONS:
- Bachelor’s degree preferably in public health, social work, counseling, and/or human services related field.
- Current certification as a Community Health Worker with at least 2 years of related experience is required.
PREFERRED QUALIFICATIONS:
- Work experience and current certification as a Peer Support Specialist (PSS), Certified Recovery Mentor (CRM), Birth Doula or another Traditional Health Worker (THW) preferred.
- Experience and knowledge of Oregon Medicaid programs and billing preferred.
- Understanding the social determinants and impacts of health and its intersections with violence, poverty, and homelessness.
- Knowledge of and experience with Oregon healthcare systems, healthcare providers, and deep understanding of the CHW model.
- Understanding Trauma-Informed Care and experience with reflective supervision.
- Must have knowledge and understanding of immigrant and refugee populations and challenges/dynamics; cultural awareness and sensitivity.
- Ability to cultivate strong working relationships with stakeholders, colleagues, and community members.
- An effective and compassionate communicator, both verbally and in writing, with excellent active listening skills to identify the needs and goals of our participants.
- Highly organized, self-directed, and with the capacity to multitask and pivot effectively; able to meet multiple and often competing deadlines.
- Strong analytical, problem solving, and decision-making skills.
- Able to respond to stressful situations balancing individual circumstances with organizational policies and program goals.
- Ability to write program reports that are accurate, clear, concise, and effectively convey program accomplishments.
- Strong skills in tracking outcomes/outputs; generating, reviewing, and/or analyzing reports.
- Strong skills and experience in tracking, reviewing and analyzing program expenses, processing program related payments and expense requests, and creating and managing complex budgets.

Positions at this level are normally responsible for a full range of supervisory responsibilities including hiring and terminations, but require Department Manager approval.
