PURPOSE OF POSITION: The Community Engagement Case Manager is responsible for identifying and reaching out to youth who are experiencing homelessness, have runaways, and are at risk of being exploited on the streets as well as through contact with other service providers. Outreach is done through both van and foot in targeted areas in Atlanta and surrounding areas. The Outreach Specialist works directly with outreach volunteers and CHGA youth ambassadors to support CHGA outreach efforts. In addition, the Outreach Specialist assists in crisis intervention on the streets and referral work with those needing services and to advocate on their behalf.

ESSENTIAL RESPONSIBILITIES:

  • Uphold Covenant House Georgia standards of conduct, policies, and procedures always.
  • Exercise and role model healthy verbal and non-verbal communication.
  • Manage the day-to-day flow of services in the Community Service Drop-in Center.
  • Advocate for youth and make referrals to appropriate services.
  • Employ milieu management techniques to heighten safety, promote structure and help youth learn healthier ways of thinking, interacting, and behaving independently and/or in group settings.
  • Provide support, encouragement, and direction to youth, peers, and volunteers.
  • Attend and participate in all scheduled or mandatory internal and external meetings, training and supervision sessions, and events as requested.
  • Collaborate with other service providers in the community.
  • Support and partner with the Outreach Volunteers and CHGA colleagues to increase efforts.
  • Conduct training with outreach volunteers and youth in collaboration with the Community Engagement Program Manager. Experience in managing and coaching volunteers.
  • Provide crisis intervention services to youth on the streets of Atlanta.
  • Create a pipeline for street outreach youth to access Drop-in services.
  • Collaborate with Program Leadership for youth outreach to the housing pipeline.
  • Communicate with supervisor and Support Services Director regarding outreach crises.
  • Ensure that CHGA is reaching its target population of youths on the streets.
  • Create case plans for youth in need of goal-setting.
  • Conduct housing assessments and strengths and needs assessments as needed.
  • Complete all necessary documentation for ETO and HMIS within 48-72 hours, and complete monthly, quarterly, and board reports. .
  • Meet with the supervisor for reflective supervision and as needed.
  • Maintain and share the monthly volunteer and event calendar.
  • Participate in other CHGA programs and activities as required.
  • Maintain and inventory outreach materials.
  • Adhere to all confidentiality requirements and standards.

KNOWLEDGE & ABILITIES

Ability to:

  • Models appropriate, professional behavior for staff to establish and maintain superior services to youth in a safe and inviting atmosphere.
  • Ensures compliance of files in accordance with specific city, state, and Federal guidelines.
  • Establish and maintain an effective working relationship with co-workers, supervisors, representatives of other agencies, and the public.
  • Effectively communicate verbally with both internal and external individuals and/or businesses
  • Lift and move items typically weighing up to 50lbs pounds.
  • Drive agency to transport items and/or persons (valid State of Georgia license required)
  • Must be computer literate with experience.
  • Performs all other duties as assigned.

WORK ENVIRONMENT/PHYSICAL DEMANDS:

  • Ability to work a flexible schedule as required including non-traditional hours, weekends, and holidays.
  • Ability to travel as needed for training or to fulfill job requirements.

 

EDUCATION & EXPERIENCE: Bachelor’s degree in human services required; a minimum of 1 to 3 years experience working with behavioral health.
