What We’re Looking For


Trillium Health Resources has a career opening for a Population Health Program Clinician to join our team! The Population Health Program Clinician is responsible for providing complex care coordination targeting those with chronic, unresolved or complex physical, behavioral health and social determinant needs. This includes providing care coordination with foundations in national evidenced based and informed standards to do whole person care. The Population Health Program Clinician will work to identify members that are appropriate for population health programs utilizing data informatics along with providing interventions and education to promote health prevention and promotion. The Program Clinician completes required documentation/paperwork/tasks into Trilliums care management platform according to timelines. 


On a typical day, you might:


  • Engage identified member into applicable Care Management program and utilizes data and information from multiple sources to address member’s needs to make clinical decisions. 
  • Identify Trillium members appropriate for population health programs; Working with Care Management and external partners to enroll and engage members by identifying mission/goals and objectives of the program along with assisting in completing a thorough clinical background.
  • Work collaboratively with members of the multidisciplinary team to develop tools and interventions designed to achieve optimal patient outcomes.
  • Coordinate and facilitate with Care Management staff and/or external partners timely implementation of assessments, care plans, and appropriate interventions for identified member populations following established policies and procedures.
  • Use a collaborative approach to assist the member with self-management goals and identifying barriers by addressing the total individual.

Qualifications


Required:

  • Master’s Degree
  • Minimum of one (1) year experience in a clinical setting with the BH/MH/SU/IDD population.
  • Fully licensed by the State of North Carolina as a LCSW, LPA, LCMHC, LCAS, LMFT or RN is required.
  • Must have a valid driver’s license.
  • Must reside within North Carolina. 
  • Must be able to travel within catchment as required.


Preferred:

  • Degree in human services field.
  • Prefer five (5) years post-Master’s experience in a clinical setting with the BH/MH/SU/IDD population and a minimum of two (2) years Maternal health and Opioid experience.
