Job Title: Clinical Transition Specialist – Day Treatment Program
Organization: Trillium Family Services
Location: Portland, Oregon
Schedule: Full-Time (40 hours/week), Monday–Friday, 8:00 AM – 4:30 PM
Salary: Starting at $68,320/year (DOE)

Position Summary

The Clinical Transition Specialist is a vital member of the interdisciplinary Day Treatment team, supporting client transitions from intake through discharge. This role is responsible for conducting comprehensive assessments, coordinating care and aftercare services, and supporting therapists with caseload management to ensure continuity of care.


Key Responsibilities

  • Conduct intake interviews and complete comprehensive psychosocial assessments
  • Develop and coordinate individualized treatment and discharge plans
  • Collaborate with therapists and interdisciplinary team members
  • Coordinate aftercare referrals (mental health, school, psychiatry, PCP, etc.)
  • Manage utilization reviews and case coordination tasks
  • Participate in daily treatment team meetings
  • Provide crisis intervention, including risk assessments and safety planning
  • Serve as liaison between program and community providers
  • Maintain a small caseload to support program needs

Qualifications

  • Master’s degree in Counseling, Social Work, Psychology, or related field required
  • 2+ years of experience in children’s mental health preferred
  • Hospital or acute care experience preferred
  • Strong assessment, care coordination, and crisis intervention skills
  • Valid driver’s license, auto insurance, and acceptable driving record
  • Proficiency with computer systems and documentation
