JOB SUMMARY:

Provide therapeutic casework to individuals, couples, families and groups in order to assist them to overcome a range of psychological and emotional concerns.  With the client as an active participant, will use treatment plans and a variety of non-medical-based treatments to address client thought processes, feelings and behavior; understand inner conflicts and find new ways to deal with and alleviate distress.  Work in collaboration with other AccessHealth and community professionals through referral and advocacy.

 

 ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

1.         Provide psychotherapy services to AccessHealth clients (to include individuals, couples, families and groups) as part of an integrated behavioral health program.

2.         Conduct a series of 10-12 sessions with identified clients to assess need, explore issues and build trust.

3.         Collaborate with other professionals to evaluate clients’ medical or physical condition (when necessary) and to assess client needs.

5.         Advocate for clients to resolve crises.

6.         Monitor, evaluate, and record client progress to include measurable goals described in treatment/care plan, modifying treatment/care plans as needed

7.         Maintain accurate and up-to-date client case records and files as well as demographics, needs, and services rendered.

8.         Produce completed encounter form for each client encounter

9.         Identify environmental impediments to client or patient progress through interviews and review of patient records.

10.       Keep abreast of developments in theory and research.

11.       Must maintain professional licensure.

12.       Provides short term behavioral health intervention to AccessHealth patients as needed

13.       Participate in behavioral health team meetings and case staffing meetings.

14.       Liaison between AccessHealth and other social services agencies.

15.       Comply with all company policies, procedures and regulatory requirements.

16.       Performs all other duties as assigned by management.

 

EDUCATION/LICENSURE/CERTIFICATION QUALIFICATIONS:

REQUIRED:

 


A Master’s Degree from an accredited college/university
Must possess a valid LCSW, LPC or LMFT


 

 

SKILLS AND EXPERIENCE:

REQUIRED:

1.         Experience in behavioral health

2.         High ethical standards.

5.         Well-organized and self-directed.

6.         Maintains strict patient confidentiality.

7.         Ability to handle multiple projects.

8.         Ability to work with people of all socioeconomic levels and cultures.

9.         Must be a consumer advocate for quality health care and service

 

 

PREFERRED:

1.         Bilingual English & Spanish preferred.

2.         Experience in a community health center preferred.

Hours:

Employees will be assigned to a regular work schedule, which may include hours before 8:00 am, after 5:00 pm, and Saturdays as determined by department needs and requirements. May travel to other AccessHealth sites to provide services as needed.
