Provides clinical supervision, consultation, and guidance to ICMS Case Managers and JCM Community Health Workers to support safe, effective, trauma-informed, harm-reduction, culturally responsive, and participant-centered service delivery. Oversees complex case reviews, Care Plans, risk and crisis response, case conferencing, documentation quality, staff development, and multidisciplinary care coordination. Establishes and maintains professional, collaborative relationships with County and City representatives, healthcare and behavioral health providers, community-based organizations, and other external partners to support coordinated services and participant outcomes. Ensures clinical practices and documentation comply with HSH ICMS and JCOD JCM contractual requirements, agency policies, and applicable legal, ethical, and professional standards.

Position Requirements:

  • Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.
  • Master’s degree in Social Work, Marriage and Family Counseling, Psychology or related field.
  • Must be registered with the California Board of Behavioral Sciences.
  • Professional Licensure: Licensed Clinical Social Worker-LCSW, Licensed Marriage and Family Therapist-LMFT, Licensed Professional Clinical Counselor-LPCC or Licensed Psychologist (Required)   
  • Must maintain a current and valid license as a mental health professional. 
  • Must have Clinical Supervision Certificate.                                                                        

Position Duties:

  • Provide regularly scheduled clinical supervision, consultation, coaching, and professional guidance to ICMS Case Managers, JCM Community Health Workers, and other designated program staff serving participants with complex medical, behavioral health, housing, justice-system, and social-service needs, consistent with HSH ICMS and JCOD JCM contractual requirements and applicable professional standards.
  • Review complex and high-acuity participant cases and provide clinical guidance regarding presenting needs, risk and protective factors, appropriate interventions, participant engagement, care coordination, referrals, crisis response, safety planning, and escalation of significant concerns.
  • Provide crisis consultation and intervention within the scope of licensure and agency protocols. Assist the Program Director with established on-call crisis coverage, including timely assessment, staff guidance, safety planning, documentation, referral, and escalation to emergency or appropriate higher-level services when indicated.
  • Review, provide clinical feedback on, and approve individualized participant Care Plans within contractually required timeframes, including at least every six (6) months or more frequently when required by the applicable program, participant acuity, identified risk, significant life events, changing needs, or progress toward established goals.
  • Facilitate and/or participate in case conferences at least monthly and more frequently as clinically or programmatically indicated. Collaborate with program staff, medical and behavioral health providers, substance-use treatment providers, housing and supportive service teams, justice-system partners, and other community providers to promote coordinated, participant-centered care.
  • Document (at minimum monthly case conferences) in EHR.
  • Provide clinical recommendations and support timely, appropriate referrals and linkages to medical, behavioral health, substance-use treatment, crisis, housing, benefits, social service, and community-based resources. Monitor whether referrals and recommended services are appropriately followed through and documented.
  • Promote trauma-informed, culturally responsive, person-centered, harm-reduction, and strengths-based practices that respect participant choice, autonomy, dignity, confidentiality, and stage of readiness. 
  • Provide clinical education, coaching, and professional development related to clinical assessment, Care Plan development, documentation, participant engagement, professional boundaries, crisis prevention and intervention, de-escalation, care
    coordination, housing stabilization, justice-system navigation, evidence-informed practices, and management of complex participant needs.
  • Monitor and support staff adherence to applicable laws, ethical and confidentiality standards, mandated-reporting obligations, licensing and scope-of-practice requirements, agency policies and procedures, and HSH ICMS and JCOD JCM program requirements.
  • Conduct ongoing clinical and documentation reviews to evaluate the quality, accuracy, timeliness, completeness, and clinical appropriateness of assessments, Care Plans, progress notes, referrals and follow-up, participant engagement efforts, safety plans, case conference activities, and discharge or transition documentation.
  • Identify participant care concerns, service gaps, barriers to engagement, safety issues, and systemic challenges. Collaborate with staff and program leadership to develop, document, and monitor appropriate interventions, corrective actions, and service-improvement strategies.
  • Support program quality assurance and continuous quality improvement through clinical supervision, case consultation, Care Plan review, documentation and chart review, identification of staff-development needs, follow-up monitoring, and evaluation of participant safety and service-delivery practices.
  • Maintain complete, accurate, timely, and confidential records of clinical supervision, consultations, Care Plan reviews and approvals, case conferences, crisis-related guidance, clinical recommendations, corrective guidance, and follow-up activities. Make records available for authorized program monitoring, audits, quality reviews, and contract compliance activities.
  • Promptly consult and coordinate with program leadership regarding significant clinical, behavioral health, medical, safety, ethical, or risk-management concerns while maintaining appropriate confidentiality and disclosing only the information necessary for effective intervention and program operations.
  • Participate in required HSH ICMS and JCOD JCM trainings, meetings, technical-assistance activities, case reviews, monitoring activities, and quality-improvement initiatives as assigned.
  • Perform other related clinical-consultation and supervisory duties consistent with the employee’s licensure, professional competence, scope of practice, agency policies, and applicable contractual requirements.
  • Performs duties in office, program-site, community, and field-based settings while interacting with agency staff, participants, family members, County and City representatives, service providers, and other community partners.
  • Regularly uses electronic health record, care-management, and data-reporting platforms, including CHAMP, CMS, and HMIS, to review participant information, monitor services, document clinical activities, and support contractual compliance. Duties require routine computer and electronic communication use while maintaining confidentiality and data security standards.
