Anchor Point Behavioral is launching Project SF, a trauma-informed individual therapy program for adults who are incarcerated, with a structured bridge into community care after release. As a clinical social worker, you will provide private one-to-one therapy inside the facility and follow participants through a phased community program, helping them move from incarceration to stable, ongoing support.
We are hiring at two levels:

  • Conduct private individual sessions of 30 to 45 minutes, based on each client’s clinical needs.
  • Complete intake assessments, including trauma history, substance use, mental health symptoms, risk, and strengths.
  • Develop individualized plans of care with measurable goals, and update them as needs change.
  • Deliver evidence-informed treatment for trauma, addiction, and co-occurring conditions.
  • Provide culturally and linguistically responsive care, including support for clients facing immigration-related stress. Refer to legal counsel for legal questions.
  • Assess and respond to suicide, violence, and safety risk following program protocols.
Bridge program
  • Continue 1 to 2 recommended sessions at release and update the plan of care for community needs.
  • Carry out the phased community plan: Phase 1 (4 months), Phase 2 (3 months, weekly), and Phase 3 (2 months, 1 to 2 sessions monthly).
  • Coordinate with the care coordinator on transportation, referrals, and linkage to housing, employment, benefits, and recovery supports.
  • Complete phase-out planning with exit recommendations, additional resources, continuing therapy options, and mentorship programs.
Collaboration and administration
  • Work with attorneys, social workers, and other healthcare professionals who refer clients.
  • Complete timely, compliant documentation in the electronic record.
  • Participate in case review, supervision, and quality and outcomes reporting.
  • Follow facility security, clearance, and confidentiality requirements.
Additional for LCSW
  • Serve as a clinical supervisor for ACSW and other associate clinicians, if qualified and assigned.
  • Handle more complex cases and assist with admission reviews.
Additional for ACSW
  • Attend weekly individual and/or group supervision and log supervised hours as required.
  • Consult with your supervisor on risk, diagnosis, and treatment planning.
  • Practice within the scope allowed for registered associates.
