The MSW (CSW II) or Licensed Clinical Social Worker (CSW III) provides psychosocial assessment, crisis intervention, therapeutic interventions consultation, education, community linkages and supportive services for Heart/VAD (Ventricular Assistive Device) transplant patients and families.  The CSW II/III coordinates resources and services in response to patients’ psychosocial needs throughout the inpatient and outpatient continuum. The LCSW has advance knowledge and skills in clinical social work and is distinguished from the MSW by leadership activities in the department, including participation in advanced program planning, provision of educational programs, clinical supervision and consultation to the clinical team

The CSW II/III provides age-appropriate assessments, interpretation of data, and delivery of interventions for the Heart/VAD Transplant program and demonstrates the ability to work with any patient regardless of race, gender, religious affiliation, cultural beliefs, lifestyle, disease process or treatment plan.

DEPARTMENT DESCRIPTION:

Dealing with an illness can be a very difficult time for patients and their loved ones for patients who are being evaluated for advanced heart therapies, including transplantation.  At UCSF Health sensitive social workers in our Social Work department are available to assist patients and their families cope with the many psychological and social problems that may arise when facing illness, hospitalization and medical treatment. 

Our social workers act as patient advocates by serving as counselors, helping to find resources and solving problems.  Any concerns related to the psychological, practical and social well-being of each patient or their loved ones may be discussed openly with our social workers.
