Summary

The Clinical Data Coordinator provides project management for a variety of clinical related projects, which include creation of reports, system selection, testing, implementation, ongoing maintenance and support. The incumbent makes judgments, coordinates project teams and takes any reasonable action required to accomplish agreed-upon project objectives. The requires the capacity to successfully manage multiple competing objectives and to communicate complex technical information in understandable terms to management and end user personnel of the medical center, its affiliated entities or authorized governmental or accrediting agencies. May require to conduct social work duties, identify and refer violent crime victims and their families to Thriving Mind South Florida for services in the VOCA Trauma Recovery Network.

Responsibilities

  • CDS coordinators, also known as resident assessment coordinators and nurse assessment coordinators, assess and evaluate the quality of emotional, mental, and physical care being given to long-term care residents. 
  • Perform the initial interview and assess new residents. 
  • From this assessment and evaluation, you will develop a care plan that has long-term and short-term goals for improvement. 
  • Assess residents at least quarterly and also update care-plan to reflect residents current clinical situation. 
  • Create significant change MDSs as needed especially for hospice residents. 
  • Conduct meetings with the multidisciplinary treatment team, caretakers, and families to discuss these goals and plans of action. 
  • MDS coordinators reach out to specialists within the community to get help in achieving care plan goals. 
  • Preparing and electronically transmitting timely reports to the national Medicare and Medicaid database. 
  • Ensuring the accuracy of medical records and coding procedures is essential for a facility to receive proper reimbursement. 
  • Become a residents' rights advocate to ensure that each resident is aware of his or her legal rights. 
  • Develop and update individualized care plans for each resident ensuring goals are attainable and approaches are realistic for resident. 
  • Annually re-do computerized care-plans (or before if needed) to give true picture of resident. 
  • Complete oral assessment on admission and annually. 
  • Update skin and fall assessments quarterly. Work with medical records to ensure diagnoses are current. 
  • Ensure all disciplines complete MDS in timely manner. 
  • Create weekly MDS/Care plan meeting list and distribute to all members of the interdisciplinary team. 
  • Employees that work for the VOCA Trauma Recovery Network may conduct social work duties to identify and refer violent crime victims and their families to Thriving Mind South Florida (contracting as South Florida Behavioral Health Network, Inc.). 
  • The duties may include but not limited to: provides brief crisis intervention to victims and/or family utilizing appropriate, evidence-based approaches, and assess eligibility for services though Thriving Mind's VOCA-funded Trauma Recovery Network (TRN); Work collaboratively with the TRN and interdisciplinary staff to implement appropriate discharge plans and facilitate warm-handoff referrals to Thriving Mind staff to link to outpatient resources; 
  • Participates in project workgroups and meetings as assigned by supervisor and/or TRN Program Director, and performs other VOCA allowable TRN-related duties as required. 

Experience

  • Generally requires 3 to 5 years of related experience.
  • At least 3 years as a Cardiovascular RN is highly preferred. Critical Care/ICU RN experience is highly preferred. 

Education

  • Bachelor's degree in related field is required

Credentials

  • Valid license or certification is required as needed, based on the job or specialty.
