Job DetailsLevel: ExperiencedJob Location: Oakland - Jefferson Office - Oakland, CA 94607Position Type: Full TimeEducation Level: Graduate DegreeSalary Range: $73,800.00 - $81,000.00 Salary/yearJob Category: Nonprofit - Social ServicesThe Clinical Case Manager will provide culturally responsive, trauma-informed mental health and community-based case management services to eligible children, youth, adults, and families in Alameda County using evidence-based treatment models. This may include providing time limited individual and group psychotherapy, case management, brief check-ins, providing group services and resource linkage. All program staff are required to maintain fidelity to program and treatment models and comply with timeliness and accuracy to documentation requirements.

Essential Responsibilities:


Deliver treatment to individuals, families, and groups to best meet the immediate need of clients.
Provide relevant services including facilitated linkages to the multiple services of Catholic Charities East Bay and to appropriate community-based resources needed to help clients meet identified goals.
Learn, adhere to, and deliver evidence-based treatment models and community clinical case management services.
Establish and maintain effective working relationships with a variety of clients from diverse cultural backgrounds.
Document all provided client services.
Conduct clinical and case management assessments.
Engage clients in the development of treatment plans to address issues identified in the assessment.
Conduct services in homes, Catholic Charities offices, or wherever is needed as determined by client needs.
Adhere to agency and program data collection requirements.
Other responsibilities as determined by program needs.


Success Metrics:


Proficient in the delivery of evidence-based treatment models used by the program
Proficient in the delivery of community-based clinical case management services.
Effective working relationships with program stakeholders.
Assessments, service and treatment plans are completed in a timely manner in accordance with program requirements.
Service delivery to clients is conducted in a culturally responsive, strengths-based, and flexible manner to continually adapt to and meet changing client needs.
Develop and maintain referral relationships with external client support agencies.


Ensure client service documentation is accurate and completed within 48 hours of client interaction.
Qualifications
Master’s Degree from an accredited college or university majoring in social work, marriage and family therapy, or clinical counseling.
A minimum of 1 year providing clinical counseling services to individuals, groups or families. Graduate school practicum placements may be counted toward years of services for new graduates.
BBS REGISTRATION: Possess a license or be license eligible as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapists (LMFT),  or Licensed Professional Clinical Counselor (LPCC), or be a registered AMFT , APCC,or ASW the California Board of Behavioral Sciences. Clinical supervision hours available.
Experience delivering trauma informed clinical case management.   This must include individual counseling, as well as family counseling or group counseling.
At least two (2) years of experience providing culturally competent, trauma-informed case management and mental health support to children, youth, adults, and families.
Excellent writing and communication skills.



Experience with restorative practices a plus.

Ability to drive to and from various office and community-based locations and possession of a valid driver’s license, proof of insurance.
Possession of a valid driver's license, proof of insurance and personal vehicle to provide services in the community including at school sites, CCEB offices, and client homes.
TB test, fingerprinting, and background check with clearance to work with children and youth.

Ability to provide therapeutic case management services linking clients to community resources.
Knowledge of laws and regulations governing mental health counseling and crisis management sufficient to guide practice with regular supervision.



Computer skills a must, including competency using a client centered database.
Ability to produce timely, clear, and concise documentation.
Willingness to participate in CCEB quality assurance activities as specified by the agency.
