We’re hiring for a full-time Clinical Care Specialist to join our Adult Services department at 41 Montebello.

 

                                               

What You’ll Be Doing

 

As a Clinical Care Specialist, you will play a central role in helping our clients’ access services and take the next steps to wellness. Your work will take you to homes, and other settings, and call upon you to collaborate with other professionals within and outside our agency to assure that clients receive the very best care available. 

 

RESPONSIBILITIES:  Under the administrative supervision of a program director, and the clinical supervision of a team lead, the Clinical Care Specialist will:

 


Maintains a caseload and conducts case management services for clinicians;
Makes appropriate referrals to address mental health and substance use needs;
Links clients and community resources effectively;
Offers behavioral health counseling and outreach;
Ensures clients attend scheduled appointments in the community and at Health Solutions;
Identifies stressors and precursors that led to the current situation or crises;
Identifies available coping mechanisms and natural supports;
Enhances problem-solving, independent living, and socialization skills among clients;
Provides support for clients to remain in home and community to avoid more restrictive living arrangements;
Maintains accurate records in the agency’s electronic health record; and
Performs other job-related duties assigned by the director or designee.


 

Physical requirements include the following:


Must be able to remain in a stationary position at least 50% of the time.
Frequently communicates with physicians about medical notes, must be able to exchange accurate information in these situations.
Constantly operates a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.
Occasionally lifts and carries items up to 20 pounds.
Occasional exposure to communicable diseases and bodily fluids.

QualificationsWhat we’re looking For—The Must-Haves


Bachelor’s degree in social work, psychology, or closely related field
Computer proficiency
Valid Colorado driver license, good driving record and access to transportation during working hours


 

What we’d Like to See in you—the Nice-to-Haves


Experience working with a multi-disciplinary team, working with adults with severe and persistent mental illness, providing case management services to adults, and community systems navigation
CAC training
Experience working with those experiencing co-occurring disorders
. Spanish language skills
