Qualifications

We are seeking:

 

A compassionate, organized Clinical Care Coordinator Per Diem serves as a vital liaison between patients, families, treatment providers, referral sources, etc. while ensuring continuation of care for patients experiencing addiction, mental health challenges, and co-occurring disorders.

 

The ideal candidate thrives in a collaborative treatment environment, works well with a team, and is dedicated to supporting each patient’s recovery journey with empathy, professionalism, and clinical excellence. This role requires a strong commitment to patient-centered care and the ability to coordinate its continuity.  This position offers an exciting opportunity for a clinical care coordinator per diem, who is eager to learn, grow, and implement continuity of care plans, aftercare and discharge planning, administrative and compliance requirements, etc.

 

The successful candidate will demonstrate compassion, adaptability, professionalism, and genuine dedication to improving the lives of patients and families impacted by addiction and mental health disorders.

 

Responsibilities:

  • Assist patients in coordinating immediate external needs, including but not limited to legal obligations, employment concerns, and disability documentation
  • Collaborate with patients and the treatment team to develop comprehensive and individualized aftercare plans
  • Maintain accurate, timely, and compliant patient interaction documentation
  • Provide consistent communication updates to treatment teams, families, and external sources
  • Promote recovery-focused care and support positive patient outcomes
  • Ensure confidentiality and uphold ethical and professional standard of care
  • Demonstrate flexibility, accountability, and commitment to quality patient care
  • Adhere to company policies and procedures

Minimum Qualifications

 

Bachelor’s degree in psychology, social work, or a related behavioral health field

 

Licensed or Certified Chemical Dependency Counselor or

Actively working towards licensure or certification, with the ability to obtain full licensure or certification within six (6) months of the start date

 

Current Basic Life Support CPR certification that includes AED training and hands-on skills assessment 

 

5+ years:

  • Experience working in mental health, addiction treatment, and/or co-occurring disorders field
  • Experience with patient-centered, trauma-informed, and recovery-oriented care
  • Experience with integrative and holistic treatment approach

Preferred Qualifications

 

Master’s degree in psychology, social work, or related a related behavioral health field

 

Licensed or Certified Chemical Dependency Counselor

 

Current Basic Life Support CPR certification that includes AED training and hands-on skills assessment 

 

10+ years:

  • Experience working in mental health, addiction treatment, and/or co-occurring disorders field
  • Experience with patient-centered, trauma-informed, and recovery-oriented care
  • Experience with integrative and holistic treatment approach
