Description

The Clinical Care Coordinator provides direct clinical services and supports individuals at the point of entry into care. This role combines care coordination, walk-in engagement, and counseling services to ensure timely, appropriate, and effective behavioral health care. The position requires strong clinical skills, flexibility, and the ability to engage clients in a fast-paced, client-facing environment.


Essential Job Duties and Responsibilities


• Provide care coordination services, including screening, referral, and linkage to appropriate treatment and support services

• Collaborate with internal teams and external partners to ensure continuity of care

• Assist clients in navigating systems of care and addressing barriers to treatment

• In coordination with agency leadership, conduct outreach to referral partners to build and maintain strong relationships, including routine follow-up and expression of appreciation to support ongoing collaboration and referral engagement.


Walk-In and Frontline Support


• Respond to walk-in clients with timely, compassionate, and professional support

• Conduct brief screenings and needs assessments to identify appropriate services

• Facilitate immediate connections to services, including crisis support when needed


Clinical Services


• Provide direct clinical services, including assessment, diagnosis, treatment planning, and counseling

• Deliver individual and/or group counseling using evidence-based practices

• Maintain accurate, timely clinical documentation in accordance with organizational, state, and federal requirements


Training and Professional Development


• Participate in ongoing training related to clinical interventions and evidence-based practices

• Apply training and newly acquired skills to service delivery

• Participate in clinical supervision and consultation as required


Additional Responsibilities


• Comply with all organizational policies, procedures, and confidentiality requirements, including HIPAA and 42 CFR Part 2

• Participate in team meetings, case staffing, and organizational initiatives

• Support data collection, reporting, and outcome measurement activities as assigned


Work Environment


• Primary duties are performed in an office-based setting, including care coordination and walk-in service delivery

• Some community-based work may be required to support outreach, engagement, and continuity of care


Qualifications

Education and Licensure

• Master’s degree in counseling, Social Work, or a related field required

• Current licensure or eligibility for licensure in the State of Texas (e.g., LPC-Associate, LPC, LCSW, LCDC)


Experience


• Experience in behavioral health and/or substance use treatment preferred

• Experience with intake processes, care coordination, or crisis response strongly preferred
