Clinic Coordinator - Center for Disabilities & Development

This position is responsible for coordinating specialized health care services for patients receiving care through the Center for Disabilities and Development (CDD) Autism Center and other multidisciplinary developmental clinics, including Neurodevelopmental, Neuromotor, Early Motor Assessment, and Spina Bifida clinics. The position develops and implements processes that support efficient patient scheduling, coordination of multidisciplinary services, and seamless transitions throughout the patient's care experience. The Specialized Care Coordinator serves as the primary point of contact for patients and families, referring providers, and the Patient Access Center to ensure timely communication and coordination of care. This position may also provide social work services and facilitate connections between patients, families, and research staff on the day of appointment.

• Consult with triage team members and referring physicians prior to patient visit to ensure appropriate clinic placement.

• Manage pre-visit documentation and communicate with patient families regarding required forms, assessments, instructions and information prior to appointments.

• Coordinate patient flow and research involvement during clinic visit with members of the multidisciplinary team, including physicians, psychologists, speech-language pathologists, audiologists, occupational therapists, social workers, research contact and other clinical staff.

• Collaborate with Social Work to assist patients and families in accessing entitlement benefits, community resources, and supportive services.

• Attend meetings involving Office of Patient Experience and respond to patient feedback.

• Respond to patient calls and inquiries regarding clinic services and care coordination.

• Provide patient families with information regarding patient resources.

• Assist in presenting professional and lay training on autism and neurodevelopmental diagnoses.

• Attend and participate in training, meetings, conferences, and workshops for the Autism Center.

• Represent and promote the services of the Center for Disabilities and Development (CDD) and related programs.

• Communicate with patient families to discuss expectations and preparations for patients visits. 

• Work within a multidisciplinary team of physicians, psychologists, therapists, social workers, and researchers in assessment of the patients or family needs.

• Respond to routine questions from patients and families regarding research participation and direct complex research questions to the research contact.

• Communicate with research contact to monitor patient visits and help to support retention of participants.

• Coordinate communication pathways with patient families prior to visit to manage expectations.

• Utilize available resources and organizational tools to achieve current and future program goals.

• Demonstrate ethical decision-making and professionalism in all interactions with patients, families, and colleagues.

• Communicate effectively with patients, families, providers, and internal and external partners.

• May assist Assistant Director, Clinical Services in hiring decisions including participation in interviews, onboarding and training of new staff.

Required Qualifications:

• Bachelor’s degree in Nursing, Social Work, or related health or human services field, or an equivalent combination of education and experience.

• Licensure or certification in related specialty area.

• 3 years of experience in patient care and/or coordination of care.

• Excellent written and verbal communication skills.

• Demonstrated attention to detail and a high level of organizational skills.

• Demonstrated proficiency with Microsoft Office products.

Desired Qualifications:

• Master’s degree in social work or equivalent combination of education and progressively responsible experience in the conduct of social work practice.

• Experience with clinical research.

• Experience and training in working with patients and families affected by autism or intellectual disability.
