Mana Resources is seeking a highly motivated, compassionate, and resourceful professional to join our team as a Client Care Coordinator This is a unique position designed for someone who is passionate about helping individuals overcome barriers and achieve greater independence.

This role combines traditional behavioral health case management with support and coordination for individuals receiving Residential Habilitation and Community-Based Rehabilitation Services (CBRS). We are looking for a true advocate and problem solver, someone who is not afraid to make phone calls, build community relationships, think creatively, and go the extra mile to help all clients access the resources they need to succeed.

Our clients come from diverse backgrounds and may experience mental illness, developmental disabilities, neurodivergence, justice system involvement, housing instability, unemployment, substance use disorders, and other complex life challenges. The ideal candidate will be comfortable working with a wide variety of populations and have a strong understanding of community resources throughout the Treasure Valley.

Position Summary
Client Care Coordinator will provide person-centered case management, resource coordination, advocacy, outreach, and support services to individuals receiving behavioral health, Residential Habilitation, and CBRS services. This position focuses heavily on connecting individuals with resources that improve quality of life, stability, independence, and overall wellness.

This position requires someone who is proactive, organized, compassionate, and able to build strong relationships with clients, providers, community partners, and support systems.

Essential Duties and Responsibilities
  • Conduct outreach and maintain regular contact with clients.
  • Complete assessments and identify barriers impacting client success.
  • Develop individualized goals and service plans.
  • Assist clients in obtaining and maintaining housing.
  • Support clients with employment searches, applications, and vocational resources.
  • Connect individuals with food assistance, transportation, financial assistance, medical care, and community support.
  • Assist clients with accessing behavioral health treatment and other healthcare services.
  • Coordinate services with Residential Habilitation staff, CBRS providers, treatment teams, probation/parole officers, guardians, family members, and community agencies.
  • Advocate for clients during meetings, appointments, and service planning.
  • Assist clients with benefits applications and maintenance, including Medicaid, SSI/SSDI, and other public assistance programs.
  • Develop and maintain relationships with community agencies and resource providers.
  • Monitor client progress and adjust service plans as needed.
  • Complete all required documentation accurately and within established timelines.
  • Maintain compliance with Medicaid, Magellan, and Idaho Department of Health and Welfare requirements.
  • Participate in team meetings, training, and ongoing professional development.
  • Provide crisis support and intervention within the scope of practice.
Populations Served
Experience working with the following populations is strongly preferred:
  • Individuals with Serious Mental Illness (SMI/SPMI)
  • Individuals with Developmental Disabilities (DD)
  • Neurodivergent individuals
  • Justice-involved populations and Department of Corrections clients
  • Individuals experiencing homelessness or housing instability
  • Individuals with co-occurring mental health and substance use disorders
  • Individuals receiving Residential Habilitation services
  • Adults and transitional-aged youth
Required Qualifications
To meet Idaho Medicaid and Magellan requirements, applicants must possess:
  • A minimum of a Bachelor's degree in a behavioral science, human services, social work, psychology, sociology, counseling, human development, criminal justice, or related field.
  • Eligibility to provide services under Idaho Medicaid and Magellan provider requirements.
  • Ability to work under supervisory protocols when required.
  • Knowledge of behavioral health systems, community resources, and case management practices.
  • Excellent organizational, communication, and documentation skills.
  • Ability to work independently while managing multiple priorities.
  • Valid Idaho driver's license and good driving record
  • Reliable transportation, current registration & automobile insurance 
  • Ability to pass enhanced criminal background 
  • CPR/First-Aid certified 
Preferred Qualifications
Preference may be given to candidates with:
  • Bilingual abilities (Spanish strongly preferred).
  • Experience as a  QIDP and/or CBRS/CM Provider
  • Experience providing behavioral health case management.
  • Experience working with individuals with developmental disabilities.
  • Experience working with justice-involved populations.
  • Knowledge of Idaho Medicaid, Magellan, CBRS, Residential Habilitation, and developmental disability systems.
  • Experience with housing resources, employment services, benefits assistance, and community resource navigation.
  • Certified Case Manager (CCM) certification or related certifications.
  • Experience with electronic health records and Medicaid documentation requirements.
