Description

I. PURPOSE/FUNCTION OF THE PROGRAM: 


Child First is an evidenced based, psychotherapeutic model that provides home-based assessment, family plan development, parent-child therapeutic intervention and education, and care coordination/case management for high risk families with pregnant people and/or children from birth to five years old, to decrease social-emotional and behavioral problems, developmental and learning problems, and abuse and neglect. This service uses a relationship-based approach that focuses on the primary attachment relationships of the young child and supports the parent, through its two-person service team, to enhance the family environment via protective, nurturing, and responsive interactions.  


II. POSITION SUMMARY: 


The Care Coordinator provides services to families in their homes and out in the community. They will reflect the cultural and linguistic needs of the population being served. Must have knowledge of the public systems relevant to the identified target population. Care Coordinators will be trained in the Child First Model. 


III. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: 


  • Collaborate with clinicians in assessment, treatment planning, and stabilization of clients.
  • Offer therapeutic support and parent guidance to families.
  • Carry a case load of 12 to 16 families; averaging 12 home visits per 40-hour work week.
  • Work individually with children as clinically indicated. 
  • Collaborate with clinicians to provide advocacy, case management and concrete services in accordance with treatment plans.
  • Advocate, mento, engage, empower, and support families to take the lead in their own Child and Family Plan of Care.
  • Help connect families to needed resources within their own community.
  • Must be willing to schedule family visits during late afternoon and evening times and/or weekend hours.
  • Attend Child First training and adhere to the Child First model as overseen and directed by the Clinical Supervisor
  • Documents and evaluates all services and procedures to be in adherence with state statutes, COA standards and Boys & Girls Village standards.
  • Manage ongoing insurance authorization for cases



IV. OTHER DUTIES AND RESPONSIBILITIES:

  • Participate in supervision and case presentations. 
  • Provides resolution of problems arising during a crisis 
  • Collaborate with other service providers to assure continuity of care.
  • Write all relevant reports, progress notes, rounds notes, and other documentation as program needs. 
  • Keeps the Clinical Supervisor apprised of all situations which involve the safety of any individuals in the program or community, to monitor the impact on the agency as well as its mission of care for children and families.
  • Prevents, intervenes and reports instances of child abuse or neglect.
  • Performs additional duties as assigned to deliver seamless services through Boys & Girls Village.


V. ESSENTIAL QUALIFICATIONS/SKILLS: 

  • BS in social work or related mental health field
  • Minimum of 3 years’ experience in Human Services, Health Care/Mental Health, Education, Community Organizing, or Advocacy is preferred. 
  • Experience working with children and adults.  
  • Experience or comfort level working with families in their homes.  
  • Must be willing to drive to off-campus sites for home visits and collaborative meetings.
  • Valid driver’s license, good driving record and reliable transportation. 


VI. PREFERRED QUALIFICATIONS: 


  •  Bi-lingual (Spanish) desired.

VIII. PHYSICAL DEMANDS:


  • Must be able to climb several flights of stairs
  • Must be able to work with clients in a variety of settings
  • Must be able to lift 35 lbs.

IX. WORKING CONDITIONS AND ENVIRONMENT


  • Evening and occasional weekends are required
  • Ability to be flexible with work schedule
