Position Summary:

Under the leadership of the Director of Administration and Operations, the PACE Center Manager is responsible for the day-to-day operations of the PACE Center and coordination of all care delivered to its participants. This includes the coordination of administrative/support staff and the interdisciplinary care team, as well as oversight of the facility. The Center Manager is the point person for emergency management.
The PACE Center Manager is a key member of the PACE leadership team, providing day-to-day support and guidance to the staff based at the PACE Center. The PACE Center Manager is also a champion of employee and participant satisfaction and places person-centered care and customer experience among his/her top priorities. This includes being regularly available at the site to interface with PACE staff and participants, fostering a customer-friendly and welcoming environment. While customer experience is a priority for every member of the Neighborhood PACE team, the Center Manager is at the forefront of customer experience.
The PACE Center Manager is a leader in the monitoring of center-based cost containment and expense oversight, working closely with the Director of Center Administration and Operations to ensure strong fiscal stewardship of PACE Center funds. In addition, the Center Manager regularly reviews and analyzes clinical, operational, and financial reports and other data to evaluate the overall performance of the IDT and its adherence to established guidelines, policies, workflows, and individual patient care plans.
The Center Manager has direct supervisory responsibilities for IDT support staff, including the assistant center manager, center secretary and well as Personal Care Attendants (PCA’s).

Essential Duties & Responsibilities:

  • Manages the PACE Center physical plant for a safe and comfortable environment.
  • Recruits, screen, hire and supervises staff including responsibility for annual performance reviews.
  • Oversees the intake assessment and care planning process to assure efficiency and completeness.
  • Engaged in activities and issues relevant to the administration and improvement of PACE Center.
  • Ensures that performance improvement and other monitoring audits are completed and reported on schedule. Reviews and acts on audit results consistent with PACE policies and procedures for ongoing performance improvement.

  • Facilitates staff meetings to ensure that staff are kept current, informed, and have the opportunity to discuss PACE policy, procedures, initiatives and events.

  • Works with project teams and PACE Center staff to successfully implement PACE priorities and initiatives at local sites.

  • Oversees the PACE Center’s Interdisciplinary Team process  to ensure that the team’s operations conform to PACE policies, procedures and standards of care including: Intake assessment and care plan development; Biannual and status change assessment and care plan updates.

  • Facilitates the daily morning meeting and IDT meetings to ensure that the team adopts an efficient and effective approach to coordinating participant care.

  • Manages cross center administrative tasks in collaboration with other Center Managers as assigned. Responsible to maintain standardized processes in collaboration with other Center Managers.

  • Is fully informed and engaged in activities and issues relevant to the administration and improvement of PACE including: PI and other projects, issues identified through committee meetings, staff meetings and data; PACE  policy and procedures; Organization-wide policy and procedure relevant to PACE

  • Works collaboratively with  PACE  managers on issues related to efficiency, quality and cost effectiveness

  • Maintains a working knowledge of all federal and state regulations relevant to PACE.

  • Demonstrates commitment to PACE mission by actively promoting the autonomy and dignity of PACE program participants.

Qualifications and Requirements:

  • College degree in a field such as rehabilitative therapy, nursing, social work, recreational therapy or healthcare administration or equivalent work experience required.
  • Supervisory and administrative experience is required. Two years’ experience and interest in working with the elderly is required.
  • Skilled facilitator with excellent communication and interpersonal skills.
  • Experience in managed care setting preferred.
  • Prior experience in interdisciplinary team setting preferred.
  • Ability to bring broad perspective on management of health and social issues to day-to-day operations.

Pay range is $80,704 - $125,000 depending on experience.
