Job Description Summary:

The Cognitive Care Network (CCN) Expansion Coordinator will serve as social worker for a caseload of individuals from the clinic and community as well as assist the Director in the strategic growth, implementation, and coordination of statewide CCN, dementia workforce development, community brain health awareness, and dementia resource-building initiatives. This position will serve as a key project manager and relationship builder, responsible for expanding partnerships, strengthening workforce capacity, supporting educational programming, and advancing innovative dementia care models throughout Kansas.

Job Description:

Job Duties

Direct Patient/Family Social Work

  • Serve as a social worker for a caseload of patients/families.

  • Assist in the development and implementation of a Social Work Cognitive Care Consult Program.

  • Serve, as part of rotation, as a consultant for the Cognitive Care Consult program.

Administration

  • Work with the Alzheimer’s Disease Research Center (ADR25C) state expansion team to ensure alignment with all Brain Health Assessment Network (BHAN) efforts.

  • Maintain records, progress reports, and performance metrics related to network expansion activities.

  • Track outcomes and community impact metrics.

  • Assist with grant-related activities, program evaluation, and sustainability planning.

  • Coordinate stakeholder engagement and focus groups to develop standardized dementia training protocols for hospital and clinic social workers and case managers.

  • Assist in the planning and coordination of a Behavioral Health Hybrid Conference focused on early detection, mood and behavioral symptom management, and counseling approaches for individuals living with cognitive impairment.

  • Assist in coordinating efforts to update, refine, and disseminate Mindful Minutes resources.

Outreach, Education, and Workforce Development

  • Assist in recruitment, coordination, and retention efforts to maximize participation in the MentorD program, targeting cohorts of up to ten (10) participants.

  • Support communication with social work education programs for Experience League presentations, Dementia Intensive (virtual and Dementia Intensive LIVE), and other social work dementia workforce initiatives.

  • Assist in developing and maintaining behavioral health partnerships that support mentorship, observational learning, and workforce development opportunities.

  • Assist in community focus groups to explore and expand programs such as Arts and Inspiration and Evening Out initiatives.

Required Qualifications

Education: Masters of Social Work or related degree/clinical licensure (Licensed Master Social Worker (LMSW).

  • This position requires a formal degree in the cited discipline area(s) to ensure that candidates have advanced knowledge, analytical skills and professional competencies necessary to perform the duties of the position.  The level of degree is commonly recognized as the standard qualification for similar roles in the public and private sector, ensuring that the university remains competitive with industry aligned practices, enhances collaboration with external partners, and supports the delivery of services and programs that meet professional and market-driven expectations.

Work Experience:

  • Six (6) months of dementia care experience including direct work with individuals identifying with memory concerns, individuals diagnosed with dementia, or families of those diagnosed.

  • Six (6) months of experience in clinical research or community outreach.

  • Experience in program coordination, healthcare, community engagement, or related field.

  • Experience building partnerships with professional stakeholders.

Preferred Qualifications

Work Experience:

  • Experience facilitating trainings, focus groups, and stakeholder meetings.

  • Experience working with Kansas healthcare, aging, and behavioral health systems.

  • Grant management or program evaluation experience.

  • Experience providing education and counseling to families facing serious health issues.

Compensation Statement:

The pay range listed for this position is determined by our compensation program using market data and salary benchmarking. A combination of factors is considered in making compensation decisions including, but not limited to, education, experience and training, qualifications relative to the requirements of the position, and funding. At the University of Kansas Medical Center, a reasonable estimate for the starting pay range will be the minimum to midpoint of the posted range, taking into account the combination of factors listed above.
