Salary Range: $48,000-$54,000

Position Description – What the Role Entails:

The CC-JAC Case Manager provides case management and support to individuals and families affected by community violence, including victims and survivors of gun violence. The Case Manager conducts intake and needs assessments, develops individualized service plans, connects participants to appropriate resources, provides follow-up and advocacy, and documents participant progress and outcomes. The Case Manager works closely with agency staff, community partners, and service providers to ensure participants receive appropriate and timely support.

Areas of Accountability - What you will be doing:

  • Conduct intake and needs assessments and develop individualized service plans with participants. 
  • Provide ongoing case management, follow-up, advocacy, and support to participants and their families. 
  • Connect participants to community resources and partner agencies based on identified needs. 
  • Follow up on referrals and monitor participant progress and outcomes. 
  • Provide assistance and resources to participants experiencing urgent needs related to violence or other 
  • circumstances. 
  • Maintain accurate and timely case notes, participant records, and required program data. 
  • Provide case updates and required information for CC-JAC reporting, monitoring, and grant compliance. 
  • Participate in CC-JAC meetings, staff meetings, trainings, and other program activities. 
  • Build relationships with community organizations, schools, faith-based organizations, and other community partners to increase awareness of and access to services. 
  • Maintain knowledge of local resources and referral options. 
  • Use required electronic case management and reporting systems, including Salesforce and agency tracking systems. 
  • Participate in community-based activities and respond to participant needs, including occasional follow-up 
  • support after community incidents. 
  • Perform other duties as assigned by the program supervisor.

Rincon’s Work Environment – What to expect:

  • Environment: Employees may occasionally work at other agency offices. Travel to and from meetings and community locations is required. 
  • Incident Response: The position may require responding to community incidents, including shootings, and working in environments affected by violence. Employees may provide on-scene or follow-up support to individuals, families, and community members affected by these events.

Schedule:

  • 8-hour shift, 1-hour unpaid lunch
  • Weekends or after-hours work may be needed

Requirements

The Knowledge and Expertise that Matters Most - What you bring to the Role:

  • Experience providing case management or related services to individuals and families, preferably in 
  • community-based services, violence prevention, victim services, or social services.
  • Understanding of trauma-informed care and the effects of community violence on individuals and families. 
  • Strong communication and interpersonal skills and the ability to build trusting relationships with participants. 
  • Knowledge of community resources and social service systems. 
  • Strong organizational skills and attention to detail, with the ability to manage multiple participants and priorities. 
  • Ability to assess needs, develop service plans, make referrals, and document participant progress. 
  • Ability to maintain confidentiality and handle sensitive information appropriately. 
  • Ability to work independently and as part of a team. 
  • Ability to work effectively with people from diverse cultural, linguistic, and socioeconomic backgrounds. 
  • Ability to use electronic case management, data entry, and reporting systems. 
  • Flexibility to work occasional evenings, weekends, or other hours based on program needs.

Position Qualifications – What is required to Perform the Role:

  • Bachelor’s degree in social work, human services, psychology, sociology, case management, or a related field preferred and/or,  
  • At least five to ten years of experience in case management, social services, community outreach, violence intervention, victim services, or a related field preferred. 
  • Knowledge of Chicago-area community resources and service providers. 
  • Ability to work effectively with individuals, families, groups, community partners, and agency staff. 
  • Ability to maintain accurate records and complete required documentation and data entry.
  • Valid driver’s license, reliable access to a vehicle, current vehicle insurance, and a safe driving record. 
  • Bilingual Spanish/English and bicultural applicants strongly preferred. 
  • Ability to work in community-based settings and provide follow-up support to individuals and families affected by violence. 
  • Ability to maintain professional boundaries and confidentiality.
