Position Summary

Help people find housing, overcome barriers, and build a stable foundation for recovery and independence.

The Parkway Center site is a homeless and transitional housing center for adult men who are experiencing homelessness in Hamilton County.  They offer Substance abuse services and case management to assist in finding permanent housing and employment.  

The Case Manager – Housing  provides hands-on support to individuals who are experiencing housing instability or need assistance obtaining and maintaining safe, stable housing.  This position works directly with clients to identify housing needs, navigate housing resources, complete applications, connect with landlords and community providers, and address barriers that may prevent successful housing placement or retention. The Case Manager serves as a knowledgeable advocate and supportive partner throughout the housing journey while also helping clients strengthen the skills and resources needed for long-term stability and recovery.

What You'll Do

  • Build supportive, trusting relationships with clients while helping them identify and achieve their housing and personal goals.

  • Assess housing needs, identify barriers to housing, and develop individualized housing-focused service plans.

  • Assist clients with housing searches, applications, documentation, waitlists, and referrals to appropriate housing programs and resources.

  • Help clients identify safe, affordable, and appropriate housing options based on their individual circumstances and needs.

  • Connect clients with emergency, transitional, permanent, supportive, and community-based housing resources as appropriate.

  • Advocate for clients with housing providers, landlords, property managers, agencies, and community partners.

  • Assist clients in navigating housing systems, eligibility requirements, applications, documentation, and other processes that can be difficult to manage independently.

  • Help clients address barriers such as lack of income, transportation, identification, credit history, rental history, budgeting, or other challenges that may impact housing opportunities.

  • Connect clients with benefits, employment resources, financial assistance, transportation, healthcare, and other supportive services that strengthen housing stability.

  • Educate clients about tenant rights and responsibilities, lease requirements, housing expectations, and strategies for maintaining successful tenancy.

  • Provide ongoing support after housing placement to help clients maintain their housing and address concerns before they become a crisis.

  • Assist with communication and problem-solving when housing-related conflicts or challenges arise.

  • Coordinate care and services with treatment providers, housing agencies, landlords, healthcare providers, and other community organizations.

  • Maintain regular contact with clients and monitor progress toward housing and recovery goals.

  • Support clients through transitions between programs, levels of care, housing settings, and community services.

  • Provide crisis intervention, advocacy, and resource coordination when needed.

  • Facilitate or participate in housing-related education, life-skills groups, workshops, and recovery activities as appropriate.

  • Participate in treatment planning and contribute to decisions regarding client services and progress.

  • Maintain timely, accurate, and thorough documentation of all housing assistance and case-management activities.

  • Enter and maintain accurate client information in electronic documentation and case-management systems.

  • Ensure documentation meets organizational, funder, clinical, and regulatory requirements.

  • Perform other duties as assigned in support of housing stability, recovery, and client success.

Successful candidates will bring:

  • A Bachelor's Degree in Behavioral Science, Social Work, Human Services, or a related field, or equivalent experience as permitted by applicable regulations.

  • A valid driver's license and auto insurance.

  • Strong knowledge of or interest in housing resources, community services, and systems that support individuals experiencing housing instability.

  • Strong interpersonal and communication skills with the ability to build relationships with clients, landlords, providers, and community partners.

  • Excellent organization and time-management skills.

  • Strong problem-solving, advocacy, and critical-thinking abilities.

  • The ability to manage multiple clients, housing opportunities, applications, deadlines, and priorities.

  • Compassion, patience, professionalism, and sound judgment.

  • The ability to work effectively with individuals experiencing substance use, housing instability, financial challenges, or other barriers to stability.

  • The ability to work independently while contributing effectively to a multidisciplinary team.

  • Flexibility and adaptability in a community-based environment.

  • Comfort using technology and electronic documentation systems; Microsoft Office proficiency preferred.
