The Targeted Case Manager Opportunity:

The Targeted Case Manager provides assessment, monitoring, care coordination, and support services for individuals with mental health needs, including those subject to court-ordered mental health commitments and settlement agreements. The position also provides Targeted Case Management services to individuals who do not require a higher level of care through Comprehensive Community Services (CCS) or the Community Support Program (CSP). The Targeted Case Manager serves as a liaison among clients, treatment providers, community resources, legal entities, and natural support systems to promote treatment engagement, recovery, safety, and community stability. Responsibilities include conducting assessments, developing crisis and service plans, coordinating and monitoring services, facilitating referrals, advocating for client needs, and maintaining communication with treatment providers and community partners. The position is responsible for monitoring adherence to court-ordered treatment requirements when applicable, documenting client progress, and preparing reports and recommendations for the court system and other stakeholders. This role requires sound professional judgment, effective communication and collaboration skills, and a working knowledge of Wisconsin mental health statutes, commitment processes, and community-based behavioral health services.

 

What You’ll Get:

  • The starting pay range for this role is $26.92 - $29.31 for Bachelor’s degree and $29.91 - $31.70 for Master’s degree based on education and experience.
  • This position in on-site and usually works weekdays during business hours.

What You Bring:

 

Required:

  • Bachelor's degree in Human Services, Psychology, Social Work, or a closely related field.
  • At least one year of experience providing mental health or behavioral health services.
  • Experience working with individuals experiencing mental health crises, serious mental illness, or complex behavioral health needs.
  • Valid Wisconsin driver's license with the ability to maintain acceptable driving privileges throughout employment.
  • Access to a personal vehicle for work-related travel.
  • Ability to meet and maintain insurability requirements under the organization's vehicle insurance policy.
  •  Ability to operate agency or personal vehicles and transport clients, patients, or staff as required by the position.

 

Preferred:

  • Experience working within community-based behavioral health programs.
  • Experience coordinating care with multidisciplinary treatment teams.
  • Familiarity with Wisconsin Chapters 34 and 51 commitment processes.
  • Experience providing case management, crisis intervention, or court-related behavioral health services

What a Targeted Case Manager does at NCHC:

 

Essential Duties:

 

  • Conduct strength-based, person-centered assessments to identify client needs, strengths, barriers, and service opportunities.
  • Monitor and support adherence to mental health commitments, settlement agreements, and court-ordered treatment requirements.
  • Coordinate referrals and connect clients to behavioral health, medical, housing, vocational, financial, and community support services.
  • Develop, implement, and maintain crisis, safety, and service plans based on assessment findings and collateral information.
  • Monitor client progress, evaluate service effectiveness, and participate in discharge and transition planning.
  • Provide services in community-based settings, including client homes and community locations, as appropriate.
  • Coordinate emergency services and crisis response interventions as needed.
  • Collaborate with Court Liaisons, Corporation Counsel, treatment providers, and other stakeholders regarding commitment and settlement agreement cases.
  • Prepare recommendations regarding the continuation, modification, or dismissal of mental health commitments.
  • Complete legal documentation, reports, and court-related correspondence in accordance with applicable requirements.
  • Testify in court proceedings as required.
  • Maintain working knowledge of Wisconsin mental health statutes and related legal processes.
  • Collaborate with multidisciplinary treatment teams, families, outpatient providers, law enforcement, probation agents, and community agencies to support continuity of care.
  • Facilitate communication among internal and external stakeholders to promote positive client outcomes.
  • Maintain accurate, timely, and complete documentation within the electronic health record and other required systems.
  • Participate in meetings, supervision, quality improvement initiatives, and required training activities.
  • Ensure compliance with applicable federal, state, accreditation, and organizational requirements.
  • Perform other duties as assigned.

 

Working Environment and Physical Requirements

 

  • Primarily works in office, community, client home, healthcare, and court-related settings.
  • Frequent travel throughout Marathon, Lincoln, Langlade, and surrounding service areas.
  • Ability to operate a motor vehicle safely and travel independently to community locations.
  • Frequent sitting, standing, walking, driving, bending, reaching, and repetitive hand movements associated with computer use and documentation.
  • Occasional lifting, carrying, pushing, and pulling of up to 25 pounds.
  • Ability to effectively communicate and observe clients in person, by telephone, and through virtual platforms.
  • Regular exposure to individuals experiencing mental health crises, emotional distress, agitation, or behavioral instability.
  • Potential exposure to communicable diseases, bodily fluids, and other healthcare-related hazards.
  • Work may occur in both indoor and outdoor environments and in varying weather conditions.
  • Ability to respond appropriately during crisis situations and maintain professional composure in stressful environments.
