Job Preview at a Glance: 

Provide Supported Housing Services to assist persons to be participants in their chosen communities in the fullest manner possible. The principles and practice of recovery are emphasized, so individuals are able to bring new meaning and an enhanced quality of life into their daily experience. Together, integration and recovery offer consumers greater opportunities to lead satisfying and productive lives.

Pay Rate: $18/hr.

Responsibilities: 

  • Performs intakes, screening and assessments on potential residents. Performs property management responsibilities, facilitates annual leases, addendums and monitor’s lease compliance, addresses lease violations and promotes housing stability across caseload assignment.
  • Processes notices of noncompliance, and facilitates eviction documentation. Coordinates and processes lease terminations, relocations, and presides in eviction hearing as needed in conjunction with Program Manager.  
  • Facilitates all housing procedures according to HUD Housing regulations, Fair Market Rents, Rent Reasonableness, and performs HQS/Habitability Standards Inspections prior to resident move in.
  • Provides Housing according to the Fair Housing Act, Arizona Residential Tenant Landlord Act and facilitates requests for reasonable accommodations according to ADA regulations.
  • Oversees caseload, and supportive service delivery. Completes and Develops Quarterly Housing Plans and Quarterly Residential Inspections. Completes residential work orders, tracks all repairs and unit turns.
  • Meets with residents and supports all facets of stabilized housing.
  • Oversees hygiene and safety standards of all units; inspects apartments, smoke detectors, fire extinguishers and unit up-keep with residents.
  • Identifies unmet needs of residents, coordinates referrals and provides follow-up.
  • Facilitates procedures for housing placement, housing exits, and unit turn oversight
  • Consults with internal staff and external case managers/service providers in establishing goals and objectives; participates in internal and external staffing’s.
  • Intervenes in crisis situations and coordinates crisis supports and interventions.
  • Consults with landlords and management companies, as needed or directed, regarding responsibilities to units and ensures that repairs are made according to contract requirements.
  • Documents all services rendered, coordinates service delivery and provides Housing Based Case management. Process and approve all Background checks according to eligibility.
  • Maintains consumer charts and eligibility documentation according to agency approved clinical standards, HUD Regulations and program policies and procedures.
  • Facilitates, complies and completes income calculations, income eligibility, zero income affidavits according to HUD income guidelines. All other duties as assigned.

Qualifications:

  • Minimum requirements consist of a Bachelor’s Degree in Social/Behavioral Sciences, Public Administration, Business, Urban Planning or related field with 1-year full-time integrated health experience.
  • Any equivalent combination of education and experience directly related to integrated health totaling five years is acceptable.
  • Housing or case management experience working with underserved or vulnerable populations preferred.
  • Negative TB results required.
  • Required to pass a 39 - month MVR Report.
  • Will be required to drive personal vehicle for business needs (as required per program requirements).
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive upon hire.
